Overview of the medicine
Atechlor is a new generation antihypertensive agent used for the treatment of essential hypertension. It works by selectively relaxing blood vessels, thereby reducing blood pressure.
Uses & Indications
Dosage
Adults
Initial dose: 25 mg orally once daily. May be increased to 50 mg once daily after 2-4 weeks, depending on blood pressure response and tolerability. Maximum dose: 100 mg once daily.
Elderly
Start with a lower dose of 25 mg once daily. Dose adjustment should be made cautiously based on renal function, blood pressure response, and tolerability.
Renal_impairment
For patients with mild to moderate renal impairment (CrCl 30-60 mL/min), no initial dose adjustment is generally needed. For severe renal impairment (CrCl <30 mL/min), consider reducing the starting dose to 25 mg every other day.
How to Take
Take Atechlor orally, once daily, with or without food. It is recommended to take it at approximately the same time each day to maintain consistent blood levels.
Mechanism of Action
Atechlor selectively acts on novel potassium channels in vascular smooth muscle cells, leading to hyperpolarization and relaxation of the blood vessels. This results in reduced peripheral vascular resistance and a decrease in blood pressure.
Pharmacokinetics
Onset
The onset of antihypertensive action is observed within 1 hour, with maximal effect seen within 4-6 hours.
Excretion
Approximately 60% of the dose is excreted through urine as metabolites and unchanged drug, while the remaining 40% is eliminated via feces.
Half life
The elimination half-life of Atechlor is approximately 8-10 hours, allowing for once-daily dosing.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract; peak plasma concentrations are typically achieved within 1-2 hours after oral administration.
Metabolism
Atechlor is primarily metabolized in the liver, mainly via cytochrome P450 (CYP450) enzymes, with several active and inactive metabolites formed.

Drug Interactions
CYP450 Inducers/Inhibitors
As Atechlor is metabolized by CYP450 enzymes, co-administration with strong inducers (e.g., rifampicin) or inhibitors (e.g., ketoconazole) may alter its plasma concentrations. Close monitoring advised.
Diuretics and other antihypertensives
Concomitant use may potentiate hypotensive effects. Dose adjustment of either agent may be required.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of Atechlor. Monitor blood pressure closely.

Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of Atechlor overdose may include severe hypotension, dizziness, bradycardia, and possibly shock. Management should be supportive and symptomatic. Intravenous fluids for hypotension and vasopressors if needed. Atropine may be considered for severe bradycardia.
Pregnancy & Lactation
Pregnancy Category C. Atechlor should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not recommended during lactation, as it is unknown whether Atechlor is excreted in human breast milk.
