Description
Overview of the medicine
Avaflut (Fluticasone Furoate) Nasal Spray is a corticosteroid medication used to treat symptoms of seasonal and perennial allergic rhinitis, such as sneezing, runny nose, nasal itching, and congestion. It works by reducing inflammation in the nasal passages.
Uses & Indications
Dosage
Adults
Two sprays in each nostril once daily (total 110 mcg daily). Once symptoms are controlled, dose may be reduced to one spray in each nostril once daily (total 55 mcg daily).
Elderly
No dosage adjustment is generally required.
Renal_impairment
No dosage adjustment is required.
How to Take
For intranasal use only. Shake gently before use. Prime the pump before first use or if not used for a week or more. Tilt head slightly forward, insert nozzle into nostril, and spray while breathing in gently.
Mechanism of Action
Fluticasone Furoate is a synthetic trifluorinated corticosteroid with potent anti-inflammatory activity. It exerts its effect by binding to glucocorticoid receptors, inhibiting the release of inflammatory mediators (e.g., histamine, leukotrienes) and cytokines, thereby reducing the inflammatory response in the nasal mucosa.
Pharmacokinetics
Onset
Initial clinical effect within 8 hours; full therapeutic effect within several days of regular use.
Excretion
Primarily excreted in feces, with a small portion eliminated renally.
Half life
Plasma half-life approximately 15-20 hours.
Absorption
Low systemic absorption (bioavailability <0.5%) due to extensive first-pass metabolism and poor gastrointestinal absorption.
Metabolism
Extensively metabolized in the liver via CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to fluticasone furoate or any of the excipients.
Drug Interactions
CYP3A4 inhibitors (e.g., ritonavir, ketoconazole)
Co-administration may increase systemic exposure to fluticasone furoate, potentially leading to systemic corticosteroid effects. Caution is advised.
Storage
Store upright at room temperature (below 30°C). Do not refrigerate or freeze. Protect from light.
Overdose
Acute overdose is unlikely to cause acute problems due to low systemic absorption. Chronic high doses may lead to systemic corticosteroid effects. Treatment is supportive.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if fluticasone furoate is excreted in human milk; use with caution in nursing mothers.
