Description
Overview of the medicine
Avloquin-H contains Hydroxychloroquine, an antimalarial and immunomodulatory drug. It is used to treat and prevent malaria, and to manage autoimmune diseases like rheumatoid arthritis and systemic lupus erythematosus.
Uses & Indications
Dosage
Adults
For Rheumatoid Arthritis/Lupus: Initial dose 200-400 mg daily, maintenance dose typically 200-400 mg daily or less. For Malaria treatment: initial 800 mg, then 400 mg at 6, 24, and 48 hours. For Malaria prophylaxis: 400 mg once weekly.
Elderly
Consider lower doses or less frequent dosing based on renal/hepatic function and overall health. Close monitoring for adverse effects.
Renal_impairment
Dosage adjustment may be necessary in patients with severe renal impairment (CrCl < 30 mL/min). Consult physician.
How to Take
Take orally with food or milk to minimize gastrointestinal upset. Should be taken as prescribed by a physician.
Mechanism of Action
Hydroxychloroquine concentrates in red blood cells, interferes with parasite growth (malaria), and inhibits lysosomal activity in immune cells, reducing inflammation and immune response in autoimmune conditions.
Pharmacokinetics
Onset
For rheumatic conditions, effects may take weeks to months to become apparent. For malaria, rapid onset.
Excretion
Primarily excreted in urine (about 25% as unchanged drug) and bile, with slow elimination due to extensive tissue binding.
Half life
Very long elimination half-life, approximately 32-50 days.
Absorption
Well absorbed from the gastrointestinal tract, bioavailability about 70-80%.
Metabolism
Extensively metabolized in the liver by CYP450 enzymes to active and inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to hydroxychloroquine or 4-aminoquinoline compounds.
- •Pre-existing maculopathy or retinal damage.
- •Long-term use in children.
Drug Interactions
Digoxin
Increased plasma concentrations of digoxin.
Antacids
May reduce absorption of hydroxychloroquine (separate administration times).
Mefloquine
Increased risk of seizures if co-administered.
Cyclosporine
Increased plasma concentrations of cyclosporine.
Insulin/Antidiabetic agents
May enhance hypoglycemic effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include headache, drowsiness, visual disturbances, cardiovascular collapse, and seizures. Management involves gastric lavage, activated charcoal, and symptomatic supportive care. Immediate medical attention is crucial.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. Excreted in breast milk; use with caution in lactating mothers, generally considered safe at therapeutic doses but potential for infant exposure.
