Description
Overview of the medicine
Cefixime is a third-generation cephalosporin antibiotic used to treat various bacterial infections, primarily in pediatric patients, such as urinary tract infections, otitis media, pharyngitis, tonsillitis, and acute bronchitis.
Uses & Indications
Dosage
Adults
Not the primary formulation for adults; adult dosage for Cefixime is typically 400 mg once daily. Consult physician for appropriate adult formulation and dosage.
Elderly
No dose adjustment generally required unless significant renal impairment is present. Monitor renal function.
Renal_impairment
Dosage adjustment is necessary for creatinine clearance < 60 mL/min. For CrCl 21-60 mL/min, 75% of standard dose. For CrCl < 20 mL/min, 50% of standard dose.
How to Take
For oral use. Reconstitute the dry powder with boiled and cooled water as per instructions on the pack. Shake the reconstituted suspension well before each use. Can be taken with or without food. Use a measuring spoon or oral syringe for accurate dosing, especially in children.
Mechanism of Action
Inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Excretion
Primarily renal (glomerular filtration and tubular secretion); also some biliary excretion.

Average 3-4 hours, prolonged in patients with renal impairment.
Absorption

Side Effects
Contraindications
- •Hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •History of severe hypersensitivity reaction (e.g., anaphylaxis) to any penicillin antibiotic.
Drug Interactions
Nifedipine
May increase cefixime bioavailability.
Probenecid
Increases cefixime plasma concentrations by decreasing renal excretion.
Carbamazepine
May increase carbamazepine levels when co-administered. Monitor carbamazepine levels.
Oral Anticoagulants (e.g., Warfarin)
May increase prothrombin time (INR) and risk of bleeding. Monitor INR closely.
Storage
Store the dry powder below 30°C, away from moisture and direct light. Once reconstituted, store the suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
In case of overdose, gastric lavage may be considered. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in human milk; use with caution during lactation.
