Description
Overview of the medicine
Avlotrin-DS 800 mg Tablet contains Metronidazole, an antibiotic effective against a wide range of anaerobic bacteria and protozoa. It is used to treat various infections including amoebiasis, giardiasis, trichomoniasis, and bacterial vaginosis.
Uses & Indications
Dosage
Adults
Dosage varies by indication. For amoebiasis: 800 mg three times daily for 5-10 days. For giardiasis: 800 mg three times daily for 5 days. For trichomoniasis: 2 g as a single dose or 250 mg three times daily for 7 days.
Elderly
No specific dose adjustment is generally required, but caution should be exercised due to potential age-related decrease in renal and hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe renal impairment or hemodialysis patients, dosage may need to be reduced.
How to Take
Take orally, preferably with or after food to minimize gastrointestinal upset. Swallow the tablet whole; do not chew or crush.
Mechanism of Action
Metronidazole enters the organism and is reduced by electron transport proteins to reactive cytotoxic compounds. These compounds bind to DNA and proteins, inhibiting nucleic acid synthesis and causing cell death.
Pharmacokinetics
Onset
Within 24 hours for most infections.
Excretion
Mainly excreted in urine (60-80%) and feces (6-15%).
Half life
Approximately 8 hours.
Absorption
Well absorbed orally; peak plasma concentrations achieved in 1-2 hours.
Metabolism
Extensively metabolized in the liver by oxidation and glucuronidation.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives.
- •First trimester of pregnancy (relative contraindication).
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia, dyspnea) when consumed with or within 3 days of Metronidazole.
Lithium
Increases serum lithium levels, leading to toxicity. Monitor lithium levels.
Warfarin
Potentiates anticoagulant effect, increasing risk of bleeding. Monitor INR closely.
Cimetidine
May prolong metronidazole half-life, increasing plasma concentrations.
Phenobarbital, Phenytoin
May decrease metronidazole plasma levels by inducing hepatic enzymes.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, and peripheral neuropathy. Management is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Metronidazole should be used during pregnancy only if clearly needed, especially avoiding the first trimester. It is excreted in breast milk; a decision should be made whether to discontinue nursing or the drug, considering the importance of the drug to the mother.
