Overview of the medicine
Avolose is an oral solution containing lactulose, a synthetic disaccharide used to treat constipation and hepatic encephalopathy. It works by drawing water into the colon, softening stools, and by reducing ammonia absorption in the gut.
Uses & Indications
Dosage
Adults
For constipation: Initially 15-30 ml daily, adjusted to response (max 60 ml/day). For hepatic encephalopathy: Initially 30-45 ml three to four times daily, adjusted to produce 2-3 soft stools per day.
Elderly
Similar to adult dosage, start with lower doses and adjust gradually.
Renal_impairment
No specific dosage adjustment required as it's minimally absorbed and primarily excreted via feces.
How to Take
Orally, can be mixed with water, fruit juice, or milk to improve palatability. Should be taken at the same time each day. For constipation, it may be taken as a single daily dose or divided into two doses. For hepatic encephalopathy, doses are typically divided throughout the day.
Mechanism of Action
Lactulose is a non-absorbable disaccharide that reaches the colon unchanged. In the colon, it is metabolized by colonic bacteria into low molecular weight organic acids, which increase osmotic pressure, leading to water retention in the colon. This softens the stool and stimulates peristalsis. In hepatic encephalopathy, it reduces blood ammonia levels by promoting ammonia diffusion from blood to gut lumen, converting ammonia to ammonium ions (which are non-absorbable), and inhibiting ammonia-producing bacteria.
Pharmacokinetics
Onset
For constipation: 24-48 hours; For hepatic encephalopathy: 24 hours to 3 days for full effect.
Excretion
Mainly excreted unchanged in feces. Small absorbed fraction excreted in urine.
Half life
Not clinically relevant due to minimal systemic absorption.
Absorption
Minimally absorbed systemically (<3%) after oral administration. Most remains in the GI tract.
Metabolism
Metabolized by colonic bacteria in the large intestine into lactic acid, acetic acid, and formic acid.
Side Effects
Contraindications

Drug Interactions
Diuretics
Increased risk of electrolyte imbalance, especially hypokalemia.
Other laxatives
Concurrent use may lead to severe diarrhea and electrolyte loss.
Non-absorbable antacids
May inhibit the action of lactulose by altering colonic pH.
Oral anti-infectives (e.g., neomycin)
May reduce the desired effect of lactulose by inhibiting lactulose-metabolizing bacteria.
Storage
Store at room temperature (below 30°C). Do not refrigerate or freeze. Keep tightly closed and out of reach of children.
Overdose
Symptoms include abdominal pain, diarrhea, and electrolyte disturbances (especially hypokalemia and hyponatremia). Treatment is symptomatic and supportive, including correction of fluid and electrolyte imbalances.
Pregnancy & Lactation
Generally considered safe during pregnancy and lactation as it is minimally absorbed. Consult a doctor before use.
