Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria. This medicine is available as a 200 mg/5 ml suspension, primarily used for pediatric patients and those who have difficulty swallowing tablets.
Uses & Indications
Dosage
Adults
Typically, 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg once daily for 4 days. For suspension, dosage varies by weight and infection type, often 10 mg/kg on day 1, then 5 mg/kg for 4 days.
Elderly
No specific dosage adjustment needed for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Caution with severe renal impairment (CrCl < 10 mL/min).
How to Take
Oral administration. Shake the suspension well before each use. Can be taken with or without food. To maximize absorption, it is often recommended to take it on an empty stomach (1 hour before or 2 hours after a meal).
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms and, thus, inhibits protein synthesis. This results in bacteriostatic activity, although at high concentrations, it may be bactericidal.
Pharmacokinetics
Onset
Clinical effect usually within 2-3 days.
Excretion
Mainly excreted unchanged in bile; a small percentage is excreted in urine.
Half life
Terminal half-life is 2-4 days (approximately 68 hours), due to extensive tissue uptake and slow release.
Absorption
Rapidly absorbed from the gastrointestinal tract; however, food significantly decreases absorption (by approximately 50%).
Metabolism
Primarily hepatic, but minimal metabolism in humans; N-demethylation is the main metabolic pathway.
Side Effects
Contraindications
- Known hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Warfarin
Increased anticoagulant effect; monitor INR closely.
Nelfinavir
Increased azithromycin serum concentrations.
Antacids (containing aluminum or magnesium)
Decreased peak serum concentrations of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacids.
QT prolonging drugs (e.g., Class IA and III antiarrhythmics, cisapride, terfenadine)
Potential for QT prolongation; avoid co-administration if possible.
Storage
Store the dry powder below 30°C in a dry place, away from light. After reconstitution, store the suspension at room temperature (15-30°C) and discard any unused portion after the specified period (e.g., 5-10 days).
Overdose
Symptoms of overdose are typically an exaggeration of known side effects: severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves supportive measures and symptomatic treatment.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. Excreted in breast milk; caution should be exercised when administered to a nursing woman.

YesClinical Trials
Extensively studied in numerous clinical trials establishing its efficacy and safety across various bacterial infections in both adult and pediatric populations. Ongoing research continues to explore its potential in other indications and resistance patterns.
Lab Monitoring
- Liver function tests (LFTs) if prolonged treatment or pre-existing hepatic impairment
- Renal function tests if prolonged treatment or pre-existing renal impairment
- INR for patients on warfarin
Doctor Notes
- Emphasize completing the full course to prevent resistance.
- Advise patients on appropriate storage and reconstitution of the suspension.
- Counsel about potential for GI upset and taking with food if needed, though absorption may be reduced.
- Be aware of potential QT prolongation, especially with concomitant medications or cardiac history.
Patient Guidelines
- Complete the full course of treatment, even if symptoms improve.
- Shake the suspension well before each use.
- Measure the dose accurately using the provided measuring device.
- Report any severe or persistent side effects to your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
Azithromycin generally does not impair the ability to drive or operate machinery. However, if side effects like dizziness or vision disturbances occur, avoid such activities.
Lifestyle Advice
- Maintain good hydration.
- Avoid alcohol consumption during treatment, as it may worsen gastrointestinal side effects.
- Take probiotics if experiencing diarrhea, under medical advice.
