Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria. This suspension form is particularly suitable for children and adults who have difficulty swallowing tablets.
Uses & Indications
Dosage
Adults
Typically, 500 mg (12.5 ml of 200mg/5ml suspension) once daily for 3 days, or 500 mg on day 1 followed by 250 mg once daily for 4 days.
Elderly
No specific dose adjustment is needed based on age alone.
Children
10 mg/kg once daily for 3 days, or 10 mg/kg on day 1 followed by 5 mg/kg on days 2-5. Maximum single dose 500 mg.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment (creatinine clearance > 10 ml/min).
How to Take
Administer orally, with or without food. For suspension, shake the bottle well before each use. Use the provided measuring device (cup/syringe) for accurate dosing.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with mRNA translation.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-3 hours.
Excretion
Mainly excreted unchanged in bile; a small amount is excreted via urine.
Half life
The terminal elimination half-life is prolonged, approximately 2-4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%.
Metabolism
Undergoes hepatic metabolism, primarily N-demethylation.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Warfarin
Increased anticoagulant effect. Monitor International Normalized Ratio (INR) regularly.
Nelfinavir
Significant increase in azithromycin serum concentrations. No dose adjustment needed for azithromycin, but monitor for known side effects.
Antacids (containing aluminum or magnesium)
May decrease peak serum concentrations; administer azithromycin at least 1 hour before or 2 hours after antacids.
QT-prolonging drugs (e.g., antiarrhythmics, cisapride, terfenadine)
Increased risk of cardiac arrhythmias. Avoid concomitant use if possible.
Storage
Store dry powder below 30°C, protected from light and moisture. Do not freeze. Keep the reconstituted suspension refrigerated (2-8°C) and discard after the specified period.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and reversible hearing loss. Management involves gastric lavage and general supportive measures as needed.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Azithromycin is excreted in breast milk; use with caution in nursing mothers.

Doctor Notes
- Emphasize the importance of completing the full course to prevent resistance.
- Counsel parents/caregivers on accurate measurement of liquid doses, especially in children.
- Be mindful of potential QT prolongation, particularly in patients with cardiac conditions or concurrent QT-prolonging medications.
Patient Guidelines
- Complete the entire course of medication as prescribed, even if symptoms improve.
- Shake the suspension bottle well before each use to ensure proper mixing.
- Report any severe or persistent side effects, especially severe diarrhea, abdominal pain, or signs of an allergic reaction.
