Uses & Indications
Dosage
Adults
Usually one Azisan Plus 40 mg Tablet once daily, preferably before breakfast. The duration of treatment depends on the condition being treated, typically 4-8 weeks for GERD.
Elderly
No specific dose adjustment is generally required for elderly patients unless severe hepatic or renal impairment is present. Use with caution.
Renal_impairment
No dose adjustment is needed for Esomeprazole in renal impairment. For Domperidone, caution is advised in severe renal impairment, and dose frequency may need to be reduced.
How to Take
Azisan Plus 40 mg Tablet should be taken orally with water, preferably at least 30 minutes before a meal. Do not crush, chew, or break the tablet; swallow it whole.
Mechanism of Action
Esomeprazole works by irreversibly blocking the H+/K+-ATPase (proton pump) in the gastric parietal cells, thereby reducing gastric acid secretion. Domperidone acts as a dopamine D2 receptor antagonist, increasing esophageal and gastric motility and acting as an antiemetic by blocking receptors in the chemoreceptor trigger zone.

Domperidone is rapidly absorbed but undergoes extensive first-pass metabolism, resulting in low absolute bioavailability (approx. 15%). Peak plasma concentrations are reached within 1 hour.
Metabolism
Esomeprazole is extensively metabolized in the liver by the cytochrome P450 system (CYP2C19 and CYP3A4). Domperidone is primarily metabolized by CYP3A4.
