Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Generally, for respiratory/skin infections, 500 mg as a single dose on Day 1, then 250 mg once daily on Days 2-5, or 500 mg once daily for 3 days. For STDs, 1 gram as a single dose.
Elderly
No specific dosage adjustment is recommended for elderly patients unless they have severe renal or hepatic impairment.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment (creatinine clearance > 10 mL/min). Use with caution in severe impairment.
How to Take
Oral administration. Shake the suspension well before each use. Can be taken with or without food. Complete the full course of therapy as prescribed.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby interfering with the translation of mRNA.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-3 hours.
Excretion
Primarily excreted unchanged in bile; minor renal excretion (<12%).
Half life
Terminal elimination half-life is 68 hours.
Absorption
Rapidly absorbed from the GI tract; bioavailability is approximately 37%. Food may decrease absorption of the suspension.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Antacids
May reduce peak plasma levels of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
Potentiates anticoagulant effect; monitor INR closely.
Nelfinavir
Increases azithromycin concentrations; monitor for known azithromycin adverse reactions.
QT-prolonging drugs
Increased risk of cardiac arrhythmias, including Torsades de Pointes (e.g., antiarrhythmics, antipsychotics).
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children. Do not freeze. Refer to product leaflet for specific storage instructions of reconstituted suspension.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Treatment is symptomatic and supportive, including gastric lavage and general supportive measures.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. Excreted in breast milk; caution should be exercised when administered to a nursing mother. Weigh benefits against risks.

- INR (if co-administered with warfarin)
- ECG (if risk factors for QT prolongation are present)
Doctor Notes
- Educate parents/caregivers on proper reconstitution (if applicable) and accurate measurement of the suspension dose for pediatric patients.
- Emphasize the importance of completing the full course of antibiotic therapy to prevent treatment failure and reduce the emergence of antibiotic resistance.
- Advise monitoring for signs of C. difficile-associated diarrhea if patients develop prolonged or severe diarrhea during or after azithromycin therapy.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor, even if symptoms improve.
- Shake the suspension well before each use and use the provided measuring device for accurate dosing.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Azithromycin may cause dizziness or visual disturbances in some individuals. If you experience these side effects, avoid driving or operating heavy machinery.
Lifestyle Advice
- Stay hydrated by drinking plenty of fluids.
- Avoid alcohol consumption as it may exacerbate gastrointestinal side effects.
