Description
Overview of the medicine
b-cef-5-mg-syrup contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections, especially in children, including respiratory tract infections, urinary tract infections, otitis media, and pharyngitis/tonsillitis. The '5 mg' strength likely refers to a dose concentration for pediatric use, where the actual per-volume concentration of Cefixime would be higher.
Uses & Indications
Dosage
Adults
Adults: Typically 400 mg once daily or 200 mg twice daily. The 5 mg strength is generally for pediatric dosage calculations.
Elderly
No dosage adjustment is generally necessary for elderly patients with normal renal function.
Renal_impairment
Dosage adjustment is recommended for severe renal impairment (CrCl < 30 mL/min). A reduction to 200 mg once daily is suggested for adults, and appropriate reduction for children based on weight.
How to Take
Administer orally, with or without food. Shake the syrup well before each use. Use a calibrated measuring device for accurate dosing.
Mechanism of Action
Cefixime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, thereby inhibiting the third and final stage of bacterial cell wall synthesis. This leads to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-6 hours.
Excretion
Primarily excreted unchanged in the urine (approximately 50-55% of absorbed dose) and via biliary/fecal routes.
Half life
Average plasma half-life is 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Absorption is not significantly altered by food.
Metabolism
Not extensively metabolized in the body. Some metabolites may be formed.
Side Effects
Contraindications
- Hypersensitivity to cefixime or other cephalosporins
- History of severe hypersensitivity reaction to penicillin (due to potential cross-reactivity)
Drug Interactions
Nifedipine
May increase cefixime peak plasma levels and AUC.
Carbamazepine
Increased carbamazepine levels when co-administered. Monitor carbamazepine levels.
Anticoagulants (e.g., Warfarin)
Increased prothrombin time and INR, potentially leading to increased bleeding risk. Monitor coagulation parameters.
Storage
Store powder below 30°C, protected from light and moisture. After reconstitution, store at room temperature (15-30°C) for 7 days or refrigerated (2-8°C) for 14 days. Do not freeze.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Supportive therapy should be administered.
Pregnancy & Lactation
Pregnancy Category B. Use only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing woman.

Lab Monitoring
- Renal function tests (for prolonged therapy or renal impairment)
- Coagulation parameters (if co-administered with anticoagulants)
Doctor Notes
- Emphasize completing the full course to prevent resistance.
- Monitor for signs of allergic reactions, especially in patients with penicillin allergy history.
- Educate parents/caregivers on proper measurement and administration of syrup.
Patient Guidelines
- Complete the full course of antibiotics, even if symptoms improve.
- Shake the syrup well before each use.
- Store at room temperature, away from moisture and heat.
- Report any severe diarrhea or allergic reactions to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
