Description
Overview of the medicine
Mupirocin 2% w/w Ointment is a topical antibacterial medication used to treat bacterial skin infections such as impetigo and folliculitis.
Uses & Indications
Dosage
Adults
Apply a small amount of ointment to the affected area 2 to 3 times daily for 5 to 10 days, or as directed by physician.
Elderly
No specific dose adjustment required for elderly patients.
Renal_impairment
No specific dose adjustment required as systemic absorption is minimal.
How to Take
For external use only. Wash hands before and after applying the ointment. Apply a thin layer to the affected skin area. The treated area may be covered with a sterile gauze dressing if desired.
Mechanism of Action
Mupirocin works by inhibiting bacterial protein synthesis. It reversibly binds to bacterial isoleucyl-tRNA synthetase, an enzyme necessary for the synthesis of bacterial proteins, thus preventing bacterial growth.
Pharmacokinetics
Onset
Clinical improvement usually seen within 3-5 days.
Excretion
The inactive metabolite (monic acid) is primarily excreted by the kidneys.
Half life
Rapidly metabolized; systemic half-life is very short (20-40 minutes).
Absorption
Systemic absorption is minimal following topical application to intact skin. Absorption may be increased with damaged skin or occlusive dressings.
Metabolism
Rapidly and extensively metabolized in the skin and plasma by hydrolysis to an inactive metabolite, monic acid.
Side Effects
Contraindications
- Known hypersensitivity to mupirocin or any of the excipients (e.g., polyethylene glycol).
- Not for ophthalmic or intranasal use.
Drug Interactions
Other topical preparations
Avoid concomitant use with other topical preparations on the same affected area, as this may dilute the mupirocin concentration and reduce its efficacy.
Storage
Store below 25°C. Do not freeze. Keep out of reach of children and direct sunlight.
Overdose
Overdose with topical application is unlikely due to minimal systemic absorption. If ingested, supportive measures should be taken.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Caution is advised during lactation as it is unknown if mupirocin is excreted in human milk.

Doctor Notes
- Advise patients on proper application technique and emphasize completing the full course of treatment to prevent resistance.
- Monitor for signs of superinfection (e.g., fungal infection) or severe allergic reactions, and discontinue if they occur.
- Avoid use near mucous membranes due to polyethylene glycol content, especially in patients with renal impairment if applied to large areas.

- Avoid contact with eyes. If contact occurs, rinse thoroughly with water.
- Do not use on large areas of skin or for prolonged periods without medical advice.
