Description
Overview of the medicine
Bazoka 5 mg Tablet is used for the treatment of erectile dysfunction (ED) and symptoms of benign prostatic hyperplasia (BPH) in adult men. It works by increasing blood flow to the penis to achieve and maintain an erection, and by relaxing muscles in the prostate and bladder for BPH symptoms.
Uses & Indications
Dosage
Adults
For ED and BPH (daily use): 5 mg once daily at approximately the same time each day. For ED (as needed): 10 mg or 20 mg at least 30 minutes before sexual activity, not more than once daily.
Elderly
No dosage adjustment required for elderly patients.
Renal_impairment
Mild to moderate: No adjustment. Severe (CrCl <30 mL/min): Not recommended for daily use, consider lower dose for as-needed use with caution.
How to Take
Take orally with or without food. Do not split, crush, or chew the tablet.
Mechanism of Action
Tadalafil is a selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5). When sexual stimulation causes local release of nitric oxide, PDE5 inhibition by tadalafil causes increased cGMP levels in the corpus cavernosum, resulting in smooth muscle relaxation and inflow of blood to the penile tissue, thus facilitating an erection. For BPH, it relaxes smooth muscles in the prostate and bladder, improving urinary flow.

Mainly excreted in feces (approximately 61%) and to a lesser extent in urine (approximately 36%) as metabolites.

Mean terminal half-life is 17.5 hours.

Primarily metabolized by CYP3A4 in the liver.
Side Effects
Contraindications
- Concomitant use with nitrates (e.g., nitroglycerin) due to risk of severe hypotension.
- Hypersensitivity to tadalafil or any component of the tablet.
- Patients with severe cardiovascular disease for whom sexual activity is inadvisable.
Drug Interactions
Nitrates
Potentiation of hypotensive effects, potentially life-threatening. Concomitant use is contraindicated.
Alpha-blockers
Increased risk of hypotension. Initiate with lowest dose of tadalafil when on stable alpha-blocker therapy.
CYP3A4 Inducers (e.g., Rifampin)
May decrease tadalafil exposure, reducing efficacy.
CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
May increase tadalafil exposure, increasing risk of adverse effects. Dose adjustment may be needed.
