Overview of the medicine
BC-Card 5 mg Tablet contains Bisoprolol Fumarate, a selective beta-1 blocker used to treat high blood pressure, angina (chest pain), and chronic stable heart failure.
Uses & Indications
Dosage
Adults
Hypertension/Angina: Initially 5 mg once daily, adjusted up to a maximum of 20 mg once daily. Chronic Heart Failure: Initially 1.25 mg once daily for one week, then gradually increased according to tolerability, up to a maximum of 10 mg once daily.
Elderly
No dosage adjustment is generally required unless there is severe renal or hepatic impairment. Start with lower doses.
Renal_impairment
For severe renal impairment (creatinine clearance < 20 mL/min), the maximum dose is 10 mg once daily.
How to Take
Take the tablet orally once daily in the morning, with or without food. Swallow whole with some liquid, do not chew or crush.
Mechanism of Action
Bisoprolol selectively blocks beta-1 adrenergic receptors in the heart, leading to a decrease in heart rate, myocardial contractility, and cardiac output, thereby reducing blood pressure and myocardial oxygen demand.

Onset
Onset of action within 2-4 hours.
Excretion
Excreted almost equally by the kidney (50% unchanged) and the liver (50% as inactive metabolites).
Half life
Plasma elimination half-life is 9-12 hours.
Absorption
Well absorbed from the gastrointestinal tract, with approximately 80% bioavailability. Peak plasma concentrations reached within 2-4 hours.
Metabolism
Approximately 50% is metabolized in the liver via CYP2D6, forming inactive metabolites.
Side Effects
Contraindications
- •Acute heart failure or decompensated heart failure requiring intravenous inotropic therapy
- •Cardiogenic shock
- •Second or third degree AV block (without a pacemaker)
- •Sick sinus syndrome
- •Sinoatrial block
- •Severe bradycardia (heart rate < 60 bpm before treatment)
- •Severe hypotension (systolic blood pressure < 100 mmHg)
- •Severe bronchial asthma or severe chronic obstructive pulmonary disease (COPD)
- •Severe forms of peripheral arterial occlusive disease or Raynaud's syndrome
- •Untreated phaeochromocytoma
- •Metabolic acidosis
Drug Interactions
Clonidine
Risk of rebound hypertension if clonidine is withdrawn abruptly while bisoprolol is continued.
Insulin and oral antidiabetic agents
Bisoprolol may mask symptoms of hypoglycemia (e.g., tachycardia, palpitations).
Calcium channel blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia, AV block, and hypotension.
Antiarrhythmics (Class I, e.g., Disopyramide, Quinidine)
May potentiate cardiodepressant effects.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include bradycardia, hypotension, bronchospasm, acute cardiac insufficiency, and hypoglycemia. Management involves symptomatic and supportive treatment; administer atropine for bradycardia, glucagon for hypoglycemia, and beta-agonists for bronchospasm.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Bisoprolol is excreted in breast milk; caution is advised when administered to a nursing mother.
