Description
Overview of the medicine
Becospray 50 mcg Nasal Spray contains Beclomethasone Dipropionate, a corticosteroid used to relieve symptoms of allergic rhinitis, such as sneezing, runny nose, nasal itching, and congestion. It works by reducing inflammation in the nasal passages.
Uses & Indications
Dosage
Adults
Adults and adolescents 12 years and over: 2 sprays (50 mcg/spray) into each nostril twice daily (total daily dose 200 mcg). Once symptoms are controlled, dose may be reduced to 1 spray into each nostril twice daily (100 mcg/day).
Elderly
No dosage adjustment required for elderly patients.
Renal_impairment
No dosage adjustment required for patients with renal impairment.
How to Take
Shake the bottle gently before each use. Prime the pump before the first use or if it has not been used for several days. Tilt head slightly forward, insert nozzle into one nostril, closing the other, and spray while sniffing gently. Repeat for the other nostril. Avoid spraying into the eyes. Clean the nozzle after use.
Mechanism of Action
Beclomethasone Dipropionate is a potent glucocorticoid with anti-inflammatory and vasoconstrictive properties. It acts by reducing the formation, release, and activity of endogenous mediators of inflammation (e.g., prostaglandins, histamine, leukotrienes) and inhibits the migration of inflammatory cells (e.g., eosinophils, mast cells) into the nasal mucosa.
Pharmacokinetics
Onset
Symptomatic relief may begin within a few hours, but full therapeutic effect may take several days (typically 3-7 days) of regular use.
Excretion
Primarily excreted via feces, with a smaller portion excreted in the urine.
Half life
The plasma half-life of the active metabolite (beclomethasone 17-monopropionate) is approximately 15 hours.
Absorption
Systemic absorption from the nasal mucosa is poor. A small amount is swallowed and undergoes extensive first-pass metabolism.
Metabolism
Extensively metabolized in the liver by esterases and cytochrome P450 enzymes (mainly CYP3A4) to its active metabolite beclomethasone 17-monopropionate and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to beclomethasone dipropionate or any excipient.
- •Untreated localized infection involving the nasal mucosa (e.g., herpes simplex).
Drug Interactions
Other Nasal Corticosteroids
Concurrent use of other nasal corticosteroids should be avoided as it may increase the risk of systemic corticosteroid side effects.
CYP3A4 Inhibitors (e.g., Ritonavir, Ketoconazole)
Co-administration with potent CYP3A4 inhibitors may increase systemic exposure to beclomethasone, potentially leading to systemic corticosteroid effects. However, the risk is generally low with nasal administration due to limited systemic absorption.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose with nasal beclomethasone is unlikely due to the low amount of active ingredient per spray and limited systemic absorption. Chronic overdose or use of higher than recommended doses may lead to systemic corticosteroid effects, including adrenal suppression. Management involves gradually reducing the dose under medical supervision.
Pregnancy & Lactation
Use during pregnancy and lactation should only be considered if the potential benefit outweighs the potential risk to the fetus or infant. Consult a doctor. Due to minimal systemic absorption, the risk to the fetus/infant is considered low.
