Overview of the medicine
Montelukast is a leukotriene receptor antagonist used for the prevention and chronic treatment of asthma, including prevention of exercise-induced bronchoconstriction, and for the relief of symptoms of seasonal and perennial allergic rhinitis. This 10 mg tablet is typically for adults and adolescents aged 15 years and older.
Uses & Indications
Dosage
Adults
For adults and adolescents 15 years of age and older: one 10 mg tablet daily, taken in the evening.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Beltas-Kids 10 mg tablet should be taken orally once daily in the evening, with or without food. It should not be used to treat acute asthma attacks.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor. The cysteinyl leukotrienes (LTC4, LTD4, LTE4) are potent inflammatory eicosanoids released from various cells, including mast cells and eosinophils. These leukotrienes bind to CysLT1 receptors in the airway and on other pro-inflammatory cells, causing bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment. Montelukast inhibits these actions, leading to reduced inflammation and improved airway function.
Pharmacokinetics
Onset
Clinical improvement in asthma symptoms typically occurs within 1 day of starting treatment.
Excretion
Montelukast and its metabolites are excreted almost exclusively via the bile into the feces (approximately 86%), with minimal renal excretion (<0.2%).
Half life
The mean plasma half-life is 2.7 to 5.5 hours in healthy young adults.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are achieved in approximately 2-4 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 enzymes CYP3A4, CYP2C9, and CYP2C8.
Side Effects
Contraindications
- Hypersensitivity to Montelukast or any component of the formulation.
- Acute asthma attacks (it is not indicated for the reversal of bronchospasm in acute asthma attacks).
Drug Interactions
Rifampin
A strong inducer of CYP3A4, 2C9, and 2C8, reduces the plasma concentration of Montelukast. Co-administration should be avoided or monitored.
Gemfibrozil
May increase the systemic exposure of Montelukast, but dosage adjustment is usually not required.
Phenobarbital
May decrease the plasma concentration of Montelukast, requiring careful monitoring.
Oral Contraceptives, Prednisone, Terfenadine, Digoxin, Warfarin
Montelukast does not have clinically significant interactions with these commonly prescribed drugs.

In cases of overdose, the most common adverse events reported were abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. There is no specific antidote. Treatment should be supportive and symptomatic.

Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Montelukast is excreted in human breast milk; therefore, caution should be exercised when administered to a nursing mother.
