Description
Overview of the medicine
Bromazepam is a benzodiazepine derivative with anxiolytic, sedative, and muscle relaxant properties, primarily used for the short-term management of anxiety and panic disorders.
Uses & Indications
Dosage
Adults
Typical dose is 1.5 mg to 3 mg, 2 to 3 times daily. In severe cases, doses may be increased to 6 mg to 12 mg, 2 to 3 times daily, under strict medical supervision.
Elderly
Initial dose of 1.5 mg once or twice daily, gradually increased if necessary, with caution due to increased sensitivity.
Renal_impairment
Reduced dosage may be required. Use with caution and monitor renal function.
How to Take
Take orally with water, with or without food. Do not chew or crush the tablet.
Mechanism of Action
Bromazepam enhances the effects of gamma-aminobutyric acid (GABA), an inhibitory neurotransmitter, by binding to benzodiazepine receptors on the GABA-A receptor complex. This increases chloride ion influx into neurons, leading to hyperpolarization and reduced neuronal excitability in the central nervous system.
Pharmacokinetics
Onset
Approximately 30-60 minutes.
Excretion
Primarily excreted in the urine as metabolites.
Half life
Elimination half-life ranges from 10 to 20 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations achieved within 1-2 hours.
Metabolism
Extensively metabolized in the liver, mainly by CYP3A4, to hydroxylated and glucuronidated metabolites, some of which are active.
Side Effects
Contraindications
- Hypersensitivity to bromazepam or other benzodiazepines
- Severe respiratory insufficiency
- Severe hepatic insufficiency
- Sleep apnea syndrome
- Myasthenia gravis
Drug Interactions
Alcohol
Increases CNS depressant effects, leading to enhanced sedation, respiratory depression, and hypotension.
Opioids
Significant risk of profound sedation, respiratory depression, coma, and death.
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin)
May increase bromazepam plasma concentrations, enhancing its effects.
Other CNS depressants (e.g., antidepressants, antihistamines)
Additive CNS depressant effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, confusion, ataxia, diminished reflexes, hypotension, respiratory depression, and in severe cases, coma. Management is supportive, including gastric lavage if recent ingestion, maintenance of airways, and monitoring vital signs. Flumazenil may be used as an antidote, but with caution due to seizure risk.
Pregnancy & Lactation
Not recommended during pregnancy, especially during the first trimester and near term, due to potential risk of congenital malformations and neonatal withdrawal symptoms. Avoid during lactation as it passes into breast milk and may cause sedation in the infant.
