Description
Overview of the medicine
Benmet is a combination medicine containing Benfotiamine and Metformin HCl. It is used to manage type 2 diabetes mellitus, particularly in patients who have or are at risk of diabetic neuropathy. Metformin helps to lower blood glucose levels, while Benfotiamine, a lipid-soluble derivative of vitamin B1, is used to prevent and treat diabetic complications like neuropathy.
Uses & Indications
Dosage
Adults
Typically, one tablet (Benfotiamine 50 mg + Metformin 500 mg) one to three times daily with meals, or as directed by the physician. Dosage should be individualized based on efficacy and tolerability, starting with a lower dose.
Elderly
Initial dosage should be conservative, monitoring renal function closely due to potential decreased renal function in elderly patients.
Renal_impairment
Contraindicated in severe renal impairment (eGFR <30 mL/min/1.73m²). Dose adjustment required for moderate impairment (eGFR 30-45 mL/min/1.73m²).
How to Take
Take orally with meals to reduce gastrointestinal side effects and to ensure better absorption.
Mechanism of Action
Metformin works by decreasing glucose production in the liver, decreasing intestinal absorption of glucose, and improving insulin sensitivity by increasing peripheral glucose uptake and utilization. Benfotiamine, a synthetic S-acyl derivative of thiamine, works by preventing the formation of advanced glycation end products (AGEs), which are harmful compounds linked to diabetic complications, particularly neuropathy.
Pharmacokinetics
Onset
Metformin: 1-2 hours for glucose lowering effect. Benfotiamine: Variable for clinical effect.
Excretion
Metformin: Primarily renal, unchanged. Benfotiamine: Renal excretion of metabolites.
Half life
Metformin: ~6.2 hours (plasma). Benfotiamine: ~3.6 hours (as thiamine).
Absorption
Metformin: Slow and incomplete absorption, 50-60% bioavailability. Benfotiamine: Better absorbed than water-soluble thiamine.
Metabolism
Metformin: Not metabolized. Benfotiamine: Metabolized to S-benzoylthiamine and then to thiamine.
Side Effects
Contraindications
- Severe renal impairment (eGFR <30 mL/min/1.73m²)
- Metabolic acidosis (including diabetic ketoacidosis)
- Hypersensitivity to Benfotiamine, Metformin, or any component of the formulation
- Acute or chronic metabolic acidosis
- Dehydration, acute myocardial infarction, septicaemia, and other conditions associated with hypoxia
Drug Interactions
Alcohol
Increases risk of lactic acidosis with Metformin.
Iodinated Contrast Agents
Temporary discontinuation of Metformin before or at the time of the procedure is recommended due to risk of renal dysfunction and lactic acidosis.
Diuretics (e.g., Thiazides)
May increase blood glucose, requiring Metformin dose adjustment.
Cimetidine, Ranolazine, Dolutegravir, Trimethoprim, Vancomycin
May increase Metformin levels by competing for renal tubular transport.
Storage
Store in a cool and dry place, below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose of Metformin can lead to lactic acidosis, which is a medical emergency. Symptoms include severe gastrointestinal upset, lethargy, muscle pain, and difficulty breathing. Benfotiamine overdose is generally well-tolerated. Treatment involves supportive measures, correction of acidosis, and hemodialysis for severe Metformin overdose.
Pregnancy & Lactation
Metformin is generally considered safe in pregnancy, but Benfotiamine safety data is limited. Use only if clearly needed and the potential benefit outweighs the potential risk. Consult a doctor before use during pregnancy or lactation.

Lab Monitoring
- Renal function (e.g., eGFR) before initiation and at least annually thereafter
- Blood glucose levels regularly
- HbA1c levels every 3-6 months
- Vitamin B12 levels periodically, especially in patients with risk factors for deficiency or on long-term therapy
Doctor Notes
- Emphasize renal function monitoring before and during therapy.
- Counsel patients on symptoms of lactic acidosis and the importance of reporting them immediately.
- Consider vitamin B12 supplementation for long-term Metformin users.
- Advise patients about the importance of lifestyle modifications alongside medication.
Patient Guidelines
- Take this medicine with meals to minimize stomach upset.
- Do not crush or chew the tablet; swallow it whole.
- Regularly monitor your blood sugar levels as advised by your doctor.
- Inform your doctor about all other medications you are taking.
- Report any unusual muscle pain, difficulty breathing, or severe stomach upset to your doctor immediately.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. Do not double the dose to catch up.
Driving Precautions
This medicine generally does not cause hypoglycemia when used alone, so it typically does not impair the ability to drive or operate machinery. However, if used in combination with other anti-diabetic agents or if you experience symptoms like dizziness or blurred vision, exercise caution.
Lifestyle Advice
- Maintain a balanced diet and engage in regular physical activity as part of your diabetes management plan.
- Limit alcohol consumption to reduce the risk of lactic acidosis.
- Stay well-hydrated.
