Description
Overview of the medicine
Besifloxacin is a fluoroquinolone antibiotic used to treat bacterial conjunctivitis caused by susceptible microorganisms.
Uses & Indications
Dosage
Adults
Instill one drop in the affected eye(s) three times a day, 4 to 12 hours apart, for 5 to 7 days.
Elderly
No dosage adjustment is necessary for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment due to minimal systemic absorption.
How to Take
For ophthalmic use only. Tilt head back, pull down the lower eyelid to create a pouch, and instill the drop into the conjunctival sac. Avoid touching the dropper tip to the eye or any other surface.
Mechanism of Action
Besifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.
Pharmacokinetics
Onset
Clinical improvement can be seen within a few days of starting treatment.
Excretion
Minimal systemic excretion.
Half life
Systemic half-life is not clinically relevant due to minimal systemic absorption.
Absorption
Minimal systemic absorption occurs following topical ophthalmic administration.
Metabolism
Minimal systemic metabolism observed.
Side Effects
Contraindications
- Hypersensitivity to besifloxacin or any other component of this medication.
- Hypersensitivity to other quinolone antibiotics.
Drug Interactions
Other ophthalmic preparations
Due to minimal systemic absorption, clinically significant systemic drug interactions are unlikely. If using other ophthalmic medications, administer them at least 5 minutes apart.
Storage
Store at 15°C to 25°C (59°F to 77°F). Protect from light. Do not freeze.
Overdose
Ophthalmic overdose is unlikely to cause systemic toxicity. If an excessive amount is instilled, the eye may be flushed with lukewarm water.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is unknown if besifloxacin is excreted in human milk; caution is advised when administered to a nursing mother.

Lab Monitoring
- Not routinely required due to topical administration and minimal systemic absorption.
Doctor Notes
- Educate patients on proper ophthalmic administration technique to ensure efficacy and prevent contamination.
- Emphasize the importance of completing the full course of therapy, even if symptoms improve early, to prevent recurrence and resistance.
- Advise patients not to wear contact lenses during treatment for bacterial conjunctivitis.
