Description
Overview of the medicine
Cefixime is a broad-spectrum, semi-synthetic, third-generation cephalosporin antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
For adults and children over 10 years or weighing more than 50 kg: 200-400 mg once daily or in two divided doses. Duration depends on the infection.
Elderly
Dosage as for adults, but renal function should be assessed, and dosage adjusted if necessary.
Renal_impairment
Creatinine clearance 21-60 mL/min: 75% of standard dose. Creatinine clearance less than 20 mL/min: 50% of standard dose.
How to Take
Bestcef can be taken orally with or without food. For oral suspension, shake well before use.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the final transpeptidation step of peptidoglycan synthesis, leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 2-6 hours.
Excretion
Primarily excreted unchanged in the urine (50-60%) via glomerular filtration and tubular secretion. Some also excreted in bile/faeces.
Half life
Average plasma half-life is 3 to 4 hours.
Absorption
Approximately 40-50% orally absorbed. Food does not significantly affect absorption.
Metabolism
Cefixime is not extensively metabolized in vivo.
Side Effects
Contraindications
- Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- History of hypersensitivity to penicillin (due to potential cross-reactivity).
Drug Interactions
Carbamazepine
Increased plasma concentrations of carbamazepine when co-administered with cefixime.
Aminoglycosides / Diuretics
Concurrent use with nephrotoxic drugs like aminoglycosides or potent diuretics may increase the risk of renal toxicity.
Anticoagulants (e.g., Warfarin)
May increase prothrombin time (PT) and International Normalized Ratio (INR), increasing bleeding risk. Monitor PT/INR carefully.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Management is primarily supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in human milk in small quantities; use with caution in nursing mothers.
