Description
Overview of the medicine
Betanem 500 mg Injection contains Meropenem, a broad-spectrum carbapenem antibiotic used to treat a wide range of severe bacterial infections.
Uses & Indications
Dosage
Adults
Typically 500 mg to 1 g IV every 8 hours, depending on the type and severity of infection. Administered by intravenous infusion over 15-30 minutes or by intravenous bolus over 3-5 minutes.
Elderly
Dosage adjustment based on renal function, similar to adults.
Renal_impairment
Dose reduction required based on creatinine clearance (e.g., lower doses or extended dosing intervals for CrCl <50 mL/min).
How to Take
Administered by intravenous infusion over 15-30 minutes or by intravenous bolus over 3-5 minutes. The powder must be reconstituted with an appropriate diluent before use.
Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell death.
Pharmacokinetics
Onset
Rapid onset of action.
Excretion
Primarily renal excretion (approximately 70% as unchanged drug in urine).
Half life
Approximately 1 hour in adults with normal renal function.
Absorption
Rapid and complete absorption after intravenous administration.
Metabolism
Minimally metabolized; primarily hydrolyzed by dehydropeptidase-I (DHP-I) in kidneys, but meropenem is resistant to DHP-I.
Side Effects
Contraindications
- Hypersensitivity to meropenem or other carbapenem antibiotics.
- History of severe hypersensitivity (e.g., anaphylactic reaction, severe skin reaction) to any other type of beta-lactam antibacterial agent (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Concomitant administration with probenecid inhibits the renal excretion of meropenem, resulting in increased plasma concentrations and a prolonged half-life of meropenem.
Valproic acid
Meropenem may decrease valproic acid plasma concentrations, leading to a loss of seizure control.
Oral anticoagulants
Concurrent use may potentiate the anticoagulant effect of warfarin and other oral anticoagulants (monitor INR).
Storage
Store below 25°C. Protect from light. Do not freeze. Reconstituted solutions should be used immediately or within a short period as directed by the manufacturer.
Overdose
In case of overdose, symptomatic and supportive treatment should be given. Meropenem can be removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Meropenem is excreted in breast milk; use with caution during lactation, only if clearly needed.
