Description
Overview of the medicine
Betavate-CL Ointment is a combination medication used to treat inflammatory skin conditions, such as eczema, dermatitis, and psoriasis, that are complicated by secondary bacterial or fungal infections. It combines a potent corticosteroid (Betamethasone Valerate) to reduce inflammation and itching, with an anti-infective agent (Clioquinol) to combat bacterial and fungal pathogens.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily, or as directed by a physician. Treatment should not exceed 7 days without medical review.
Elderly
No specific dosage adjustment is generally required, but caution is advised in elderly patients, especially with extensive use due to potential for increased absorption.
Renal_impairment
No specific dosage adjustment is necessary for topical application, but systemic absorption should be minimized.
How to Take
For topical use only. Wash hands before and after application. Apply a thin film to the affected skin area and gently rub it in. Avoid contact with eyes, nose, and mouth.
Mechanism of Action
Betamethasone Valerate is a potent corticosteroid that works by activating natural substances in the skin to reduce swelling, redness, and itching. Clioquinol is an iodine-containing hydroxyquinoline derivative with broad-spectrum antibacterial and antifungal properties. It acts by chelating metal ions, which are essential for microbial growth, and by inhibiting microbial enzyme systems.
Pharmacokinetics
Onset
Anti-inflammatory and antimicrobial effects begin within hours to days of initial application.
Excretion
Systemically absorbed Betamethasone and its metabolites are excreted primarily via the kidneys. Clioquinol metabolites are excreted mainly in urine.
Half life
Not well-established for topical application; systemically absorbed corticosteroids have varying half-lives (e.g., Betamethasone ~3-5 hours biological half-life).
Absorption
Systemic absorption of Betamethasone Valerate from topical application is generally minimal, but can increase with prolonged use, application to large surface areas, broken skin, or with occlusive dressings. Clioquinol absorption is also minimal from intact skin.
Metabolism
Systemically absorbed Betamethasone is primarily metabolized in the liver. Clioquinol is also metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to Betamethasone, Clioquinol, or any other component of the ointment
- •Viral infections of the skin (e.g., herpes simplex, varicella)
- •Fungal infections of the skin without a bacterial component
- •Tuberculosis of the skin
- •Acne vulgaris, rosacea, perioral dermatitis
- •Perianal and genital pruritus
- •Ulcerated skin areas, wounds
- •Children under 1 year of age
Drug Interactions
No significant systemic interactions
Due to minimal systemic absorption, clinically significant drug interactions are unlikely with topical use. However, prolonged use on large areas might lead to some systemic absorption. If systemic absorption occurs, interactions typical of corticosteroids (e.g., with antidiabetics, antihypertensives) could theoretically be relevant.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose with topical corticosteroids is unlikely. However, prolonged or excessive use of Betavate-CL, especially on large areas or under occlusive dressings, may lead to systemic effects such as Cushing's syndrome, hyperglycemia, or adrenal suppression. Management involves gradual withdrawal of the steroid.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Avoid prolonged use, large amounts, or application to extensive body areas during pregnancy. Caution should be exercised when administering to a nursing mother. Consult a doctor before use.
