Bethanechol is a direct-acting cholinergic agonist primarily used to stimulate the bladder and treat non-obstructive urinary retention.
Uses & Indications
Dosage
Adults
Initial dose is usually 10-25 mg three to four times daily. Dosage may be adjusted based on patient response, up to 50 mg three to four times daily. Administer on an empty stomach.
Elderly
Start with lower doses (e.g., 5-10 mg three to four times daily) due to increased sensitivity and potential for adverse effects.
Renal_impairment
Use with caution; dosage adjustment may be needed due to primary renal excretion. Close monitoring is recommended.
How to Take
Administer orally, preferably on an empty stomach (one hour before or two hours after meals) to minimize nausea or vomiting. Take with a full glass of water.
Mechanism of Action
Bethanechol selectively stimulates muscarinic receptors, particularly M3 receptors in the bladder detrusor muscle, leading to increased bladder tone, contraction of the detrusor muscle, and relaxation of the trigone and external sphincter, thereby promoting micturition.

Side Effects
Contraindications
- •Hypersensitivity to bethanechol or any component of the formulation.
- •Mechanical obstruction of the gastrointestinal or urinary tract.
- •Hyperthyroidism, peptic ulcer, bronchial asthma, cardiac disease (e.g., severe bradycardia, AV conduction defects, hypotension, coronary artery disease), epilepsy, parkinsonism.
- •Recent myocardial infarction, conditions where increased gastrointestinal or urinary tract motility might be harmful (e.g., bladder neck obstruction, spastic gastrointestinal disturbances), peritonitis, acute inflammatory lesions of the gastrointestinal tract.
Drug Interactions
Quinidine, Procainamide
May antagonize the cholinergic effects of bethanechol.
Cholinesterase inhibitors
Concurrent use may lead to additive cholinergic effects and increased toxicity.
Ganglionic blocking agents
Severe abdominal symptoms and a critical drop in blood pressure may occur.
Atropine and other anticholinergic drugs
Antagonize the effects of bethanechol.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, abdominal cramps, diarrhea, salivation, sweating, flushing, bradycardia, hypotension, and circulatory collapse. Treatment involves discontinuation of the drug and administration of atropine sulfate (0.6 mg subcutaneous or intravenous) as an antidote. Supportive measures should be provided.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether bethanechol is excreted in human milk; use with caution in nursing mothers.
