Description
Overview of the medicine
Bexsul-6-3 Ointment is a topical medication combining a corticosteroid (Betamethasone Valerate), an antibiotic (Neomycin Sulfate), and a protective agent (Zinc Oxide). It is used to treat inflammatory skin conditions such as eczema or dermatitis where bacterial infection is present or suspected, and to soothe irritated skin.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected area 2-3 times daily, or as directed by physician. Do not use for more than 7-10 days without medical review.
Elderly
Same as adult dosage; caution with prolonged use due to skin thinning.
Renal_impairment
No specific dose adjustment for topical use, but prolonged or extensive use should be monitored.
How to Take
For external use only. Wash hands before and after application. Apply a thin layer to the affected skin area and rub gently until absorbed. Do not apply on open wounds or mucous membranes. Avoid contact with eyes.
Mechanism of Action
Betamethasone Valerate is a potent corticosteroid that suppresses the inflammatory response. Neomycin Sulfate is an aminoglycoside antibiotic that inhibits bacterial protein synthesis, effective against a wide range of gram-negative and some gram-positive bacteria. Zinc Oxide provides mild astringent and protective properties, soothing and drying the affected skin area.
Pharmacokinetics
Onset
Anti-inflammatory effects typically within hours; antibacterial effects within days.
Excretion
Excreted primarily via urine and bile if systemically absorbed.
Half life
Not precisely determined for topical application; systemic half-life of Betamethasone is about 5-6 hours.
Absorption
Minimal systemic absorption through intact skin; increased with inflamed or damaged skin, occlusion, or prolonged use.
Metabolism
Mainly hepatic metabolism for systemically absorbed Betamethasone; Neomycin is poorly absorbed.
Side Effects
Contraindications
- •Hypersensitivity to Betamethasone, Neomycin, Zinc Oxide or any component of the ointment
- •Fungal, viral (e.g., herpes simplex, vaccinia, varicella), or parasitic skin infections (untreated)
- •Tuberculosis of the skin
- •Acne rosacea
- •Perioral dermatitis
- •Ulcerative conditions
- •Children under 2 years (due to Neomycin absorption risk and corticosteroid effects)
Drug Interactions
Other Aminoglycosides
Prolonged or extensive use may lead to systemic absorption of Neomycin, potentially increasing ototoxicity/nephrotoxicity with other aminoglycosides.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Keep out of reach of children.
Overdose
Overdose from topical application is unlikely but chronic overuse or misuse can lead to systemic corticosteroid effects (e.g., Cushing's syndrome, HPA axis suppression) and potential neomycin toxicity (ototoxicity, nephrotoxicity) if absorption is significant. Treatment is symptomatic and supportive; gradual withdrawal of the corticosteroid may be necessary.
Pregnancy & Lactation
Use with caution during pregnancy only if the potential benefit outweighs the potential risk to the fetus, and under strict medical supervision. Avoid extensive or prolonged use. For lactation, systemic absorption is minimal, but caution is advised; avoid applying to the breast area.
