Overview of the medicine
Bicalutamide is a non-steroidal antiandrogen used primarily in the treatment of prostate cancer. It works by blocking androgen receptors, preventing androgens like testosterone from stimulating prostate cancer cells.
Uses & Indications
Dosage
Adults
For advanced prostate cancer: 50 mg orally once daily. For locally advanced prostate cancer: 150 mg orally once daily.
Elderly
No dose adjustment required.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment. Caution with severe impairment.
How to Take
Take orally, with or without food, at the same time each day.
Mechanism of Action
Bicalutamide is a non-steroidal antiandrogen (NSAA) that selectively binds to androgen receptors in target tissues, blocking the binding and activity of natural androgens (testosterone and dihydrotestosterone). This prevents the growth stimulation of androgen-sensitive prostate cancer cells.
Pharmacokinetics
Onset
Not acutely an 'onset' drug; therapeutic effects are seen over weeks of continuous use.
Excretion
Excreted in the urine and feces in approximately equal proportions as metabolites.
Half life
Approximately 5.8 days (R-enantiomer, which is active).
Absorption
Well absorbed after oral administration, with peak plasma concentrations occurring within 3-5 hours. Food does not significantly affect absorption.
Metabolism
Extensively metabolized in the liver, primarily by hydroxylation and glucuronidation. The (R)-enantiomer is stereospecifically metabolized.
Side Effects
Contraindications
- •Hypersensitivity to bicalutamide or any component of the formulation.
- •Women, particularly pregnant or lactating women.
- •Children.
Drug Interactions
Warfarin
Increased anticoagulant effect, requiring close monitoring of INR.
Midazolam
Increased midazolam exposure, caution with co-administration.
Cyclosporine
Potential for increased cyclosporine levels, monitor renal function and cyclosporine levels.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
There is no specific antidote for bicalutamide overdose. Treatment should be symptomatic and supportive. Dialysis is unlikely to be beneficial.
Pregnancy & Lactation
Contraindicated in pregnant and lactating women. Bicalutamide can cause fetal harm. Not for use in women.
