Description
Overview of the medicine
Biocal-Kit 150 mg tablet is a dietary supplement formulated to provide essential calcium and Vitamin D3. It is primarily used to support bone health, prevent and treat calcium deficiencies, and as an adjunct in the management of osteoporosis. The 150 mg refers to the elemental calcium content.
Uses & Indications
Dosage
Adults
Typically 1-2 tablets daily, or as directed by a physician. Should be taken with a meal.
Elderly
Same as adult dosage, but caution with renal function. Consult physician.
Renal_impairment
Use with caution and monitor serum calcium and phosphate levels. Dosage adjustment may be necessary under medical supervision.
How to Take
Oral administration. Tablets should be swallowed whole with water, preferably with meals to enhance absorption and reduce gastrointestinal discomfort.
Mechanism of Action
Calcium is fundamental for the development and maintenance of healthy bones and teeth, as well as for proper nerve, muscle, and heart function. Vitamin D3 (Cholecalciferol) is crucial for regulating calcium and phosphate levels in the body, enhancing the absorption of calcium from the intestine, and promoting its deposition into bone matrix.
Pharmacokinetics
Onset
Therapeutic effects on bone density are gradual, over weeks to months. Immediate effects on serum calcium are observed within hours.
Excretion
Calcium is excreted mainly via feces (unabsorbed fraction) and urine. Vitamin D metabolites are primarily excreted in bile and feces, with some renal excretion.
Half life
Elemental calcium has a complex pharmacokinetic profile. Cholecalciferol has a half-life of about 24 hours in serum; its active metabolite, calcitriol, has a half-life of 3-6 hours.
Absorption
Calcium is absorbed primarily in the small intestine via active transport (Vitamin D dependent) and passive diffusion. Cholecalciferol is fat-soluble, absorbed in the small intestine, and transported via lymphatic system.
Metabolism
Calcium is not metabolized. Cholecalciferol is hydroxylated in the liver to 25-hydroxyvitamin D3 (calcifediol) and then in the kidneys to the active form, 1,25-dihydroxyvitamin D3 (calcitriol).
Side Effects
Contraindications
- Hypercalcemia (abnormally high levels of calcium in the blood)
- Hypercalciuria (excessive calcium in the urine)
- Severe renal impairment
- Hypervitaminosis D (excessively high levels of Vitamin D)
- Nephrolithiasis (kidney stones) or history of recurrent kidney stones
Drug Interactions
Bisphosphonates
Calcium can interfere with absorption; separate administration by at least 30 minutes to 2 hours.
Corticosteroids
Can reduce calcium absorption and increase calcium excretion, potentially necessitating higher calcium/Vitamin D doses.
Thiazide diuretics
May reduce calcium excretion, increasing risk of hypercalcemia.
Tetracyclines & Quinolones
Calcium can reduce absorption of these antibiotics. Take at least 2-3 hours before or 4-6 hours after calcium.
Thyroid hormones (Levothyroxine)
Calcium may impair absorption. Administer at least 4 hours apart.
Storage
Store in a cool, dry place (below 30°C), away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, thirst, polyuria, muscle weakness), and in severe cases, cardiac arrhythmias, kidney stones, and renal failure. Treatment involves discontinuing the supplement, hydrating the patient, and managing hypercalcemia.
Pregnancy & Lactation
Safe when used within recommended daily allowances. Pregnant and lactating women have increased calcium and Vitamin D requirements, but doses should be determined by a healthcare provider to avoid excessive intake.
