Description
Overview of the medicine
Bislol 7.5 mg Tablet contains Bisoprolol Fumarate, a beta-blocker used to treat high blood pressure, angina, and chronic stable heart failure. It works by slowing down the heart rate and relaxing blood vessels.
Uses & Indications
Dosage
Adults
Hypertension/Angina: Initially 5 mg once daily, may be increased to 10 mg once daily. Max 20 mg once daily. Heart Failure: Initially 1.25 mg once daily, titrate up slowly over weeks to a maximum of 10 mg once daily, as tolerated.
Elderly
No specific dose adjustment is usually required unless there is significant renal or hepatic impairment. Start with lower doses and titrate carefully.
Renal_impairment
For severe renal impairment (creatinine clearance < 20 mL/min), or severe hepatic impairment, do not exceed 10 mg once daily. Start with 2.5 mg or 5 mg once daily.
How to Take
Take the tablet orally once daily in the morning, with or without food. Swallow whole with water, do not chew or crush. Do not stop taking it suddenly.
Mechanism of Action
Bisoprolol is a highly beta-1 selective adrenergic receptor blocker. It selectively blocks beta-1 adrenergic receptors in the heart, reducing heart rate, myocardial contractility, and cardiac output, leading to decreased blood pressure. It also reduces renin release from the kidneys.

Side Effects
Contraindications
- Acute heart failure or during episodes of decompensated heart failure requiring intravenous inotropic therapy
- Cardiogenic shock
- Second or third degree AV block (without a pacemaker)
- Sick sinus syndrome (without a pacemaker)
- Severe bradycardia (heart rate < 60 bpm before treatment)
- Severe asthma or severe chronic obstructive pulmonary disease (COPD)
- Severe peripheral arterial occlusive disease or Raynaud's syndrome
- Phaeochromocytoma (untreated)
- Metabolic acidosis
- Hypersensitivity to bisoprolol or any components of the tablet
Drug Interactions
NSAIDs (e.g., ibuprofen)
May reduce the hypotensive effect of bisoprolol.
Antiarrhythmics (e.g., disopyramide, amiodarone)
Increased risk of cardiac depression and bradycardia.
Calcium channel blockers (e.g., verapamil, diltiazem)

Symptoms of overdose include bradycardia, hypotension, bronchospasm, and acute cardiac insufficiency. Management involves symptomatic and supportive treatment; gastric lavage, activated charcoal, IV fluids, atropine for bradycardia, glucagon, or other interventions as needed.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Bisoprolol is excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor.
