Description
Overview of the medicine
Blextin 10 mg tablet contains Bilastine, a non-sedating antihistamine used to relieve symptoms of allergic rhinoconjunctivitis (seasonal and perennial) and urticaria (hives). It is primarily indicated for use in children aged 6 to 11 years.
Uses & Indications
Dosage
Adults
The standard adult dose for Bilastine is 20 mg once daily. For children aged 12 years and above, 20 mg once daily. For the 10 mg tablet, adult use is typically not the standard therapeutic dose unless directed by a physician for specific circumstances or if two 10 mg tablets are prescribed to achieve the 20 mg dose.
Elderly
No dose adjustment is generally required for elderly patients. Experience in patients over 65 years is limited.
Children
10 mg once daily for children aged 6 to 11 years (with a body weight of at least 20 kg). The tablet should be swallowed with water.
Renal_impairment
No dose adjustment is generally required for adults and adolescents with renal impairment. For children with renal impairment, consult a pediatrician.
How to Take
Take the tablet orally once daily. It should be taken on an empty stomach, either one hour before or two hours after food or fruit juice (especially grapefruit juice), as food reduces the bioavailability of bilastine.
Mechanism of Action
Bilastine acts as a selective peripheral H1-receptor antagonist, blocking the effects of histamine and thereby reducing allergic symptoms such as sneezing, runny nose, itchy eyes, and skin rashes.

Onset of action typically occurs within 30-60 minutes.
Excretion
Approximately 95% of the dose is excreted unchanged, with about 67% via feces and 28% via urine. Renal and hepatic impairment do not significantly affect elimination.
Half life
Mean elimination half-life is approximately 14.5 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Tmax) is reached in approximately 1.3 hours. Absolute oral bioavailability is about 61%. Food (especially grapefruit juice) significantly reduces bioavailability.
Metabolism
Bilastine is not significantly metabolized in humans. It is not an inducer or inhibitor of CYP450 isoenzymes.

Contraindications
- Hypersensitivity to bilastine or any of the excipients of the tablet.
Drug Interactions
Diltiazem
Increases plasma levels of bilastine (due to P-gp inhibition).
Ritonavir
Possible increase in bilastine exposure.
Cyclosporine
Possible increase in bilastine exposure.
Erythromycin
Increases plasma levels of bilastine (due to P-gp inhibition).
Ketoconazole
Increases plasma levels of bilastine (due to P-gp inhibition).

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment is recommended. There is no specific antidote for bilastine. Gastric lavage and activated charcoal may be considered if ingestion was recent. Monitor vital signs and clinical status.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harmful effects on pregnancy or fetal development. However, data from pregnant women are limited. Use during pregnancy only if clearly needed and the potential benefit outweighs the potential risk. It is unknown whether bilastine is excreted in human breast milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
