Description
Overview of the medicine
Boncare 150 mg tablet contains Risedronate Sodium, a bisphosphonate medication used to strengthen bones and treat or prevent osteoporosis, especially in postmenopausal women and men.
Uses & Indications
Dosage
Adults
For osteoporosis: 150 mg orally once a month. The tablet should be taken on the same day each month.
Elderly
No specific dose adjustment is considered necessary for elderly patients.
Renal_impairment
Not recommended for patients with severe renal impairment (creatinine clearance < 30 mL/min). No dose adjustment is needed for mild to moderate renal impairment (creatinine clearance ≥ 30 mL/min).
How to Take
Take Boncare 150 mg tablet with a full glass (120-180 mL) of plain water at least 30 minutes before the first food, drink (other than plain water), or medication of the day. Do not chew or suck the tablet. Remain in an upright position (sitting or standing) for at least 30 minutes after taking the tablet and until after the first food of the day.
Mechanism of Action
Risedronate inhibits osteoclast-mediated bone resorption by binding to hydroxyapatite crystals in bone, thereby decreasing bone turnover and increasing bone mineral density.
Pharmacokinetics
Onset
Clinical effects on bone mineral density are observed within months of initiation, with fracture risk reduction typically seen within 1-2 years.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in urine within 24 hours. Unabsorbed drug is excreted in feces.
Half life
The terminal elimination half-life from bone is prolonged, estimated to be approximately 480 hours (20 days).
Absorption
Low oral absorption, with an average bioavailability of about 0.63% (for 30 mg dose), further reduced by food.
Metabolism
Risedronate is not metabolized in humans.
Side Effects
Contraindications
- Hypocalcemia
- Known hypersensitivity to risedronate or any component of the formulation
- Esophageal abnormalities which delay esophageal emptying (e.g., stricture, achalasia)
- Inability to stand or sit upright for at least 30 minutes
Drug Interactions
Aspirin/NSAIDs
Concomitant use with NSAIDs has been associated with an increased risk of upper gastrointestinal adverse effects.
Calcium supplements and antacids
Concomitant administration significantly interferes with the absorption of risedronate. These products should be taken at a different time of day.
Storage
Store in a cool and dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypocalcemia, hypophosphatemia, and upper gastrointestinal adverse events (e.g., esophageal ulcer, dyspepsia). Management involves milk or antacids to bind risedronate and relieve symptoms, and monitoring serum calcium levels.
Pregnancy & Lactation
Pregnancy Category C. Risedronate should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether risedronate is excreted in human milk; caution should be exercised when Boncare is administered to a nursing woman.
