Description
Overview of the medicine
Bonex-D 500 mg Tablet is a combination of calcium carbonate and vitamin D3 used to supplement calcium and vitamin D deficiencies, promote bone health, and prevent osteoporosis.
Uses & Indications
Dosage
Adults
Usually 1-2 tablets daily, or as directed by a physician.
Elderly
Same as adult dosage, or as directed by a physician, with careful monitoring for hypercalcemia.
Renal_impairment
Use with caution; dose adjustment may be necessary based on serum calcium and phosphate levels. Not recommended in severe renal impairment without medical supervision.
How to Take
Take orally with water, preferably with a meal to enhance absorption. Do not chew or crush the tablet.
Mechanism of Action
Calcium is essential for bone formation, muscle contraction, and nerve function. Vitamin D3 helps the body absorb calcium from the gastrointestinal tract and regulate calcium and phosphate levels in the body, promoting mineralization of bone.
Pharmacokinetics
Onset
Therapeutic effects (e.g., bone density improvement) are long-term, not immediate.
Excretion
Calcium is primarily excreted via kidneys and feces. Vitamin D metabolites are primarily excreted in bile and feces, with some renal excretion.
Half life
Elemental calcium does not have a single half-life. Vitamin D3 (Cholecalciferol) has a half-life of about 24-48 hours, while its active metabolite calcitriol has a longer half-life.
Absorption
Calcium absorption is variable, influenced by vitamin D levels and dietary factors. Vitamin D3 is well absorbed from the small intestine.
Metabolism
Calcium is not metabolized significantly. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D [25(OH)D] and then in the kidneys to 1,25-dihydroxyvitamin D [1,25(OH)2D] (calcitriol), the active form.
Side Effects
Contraindications
- Hypersensitivity to calcium carbonate, vitamin D3, or any component of the formulation.
- Hypercalcemia (high calcium levels in the blood).
- Hypercalciuria (high calcium levels in the urine).
- Severe renal impairment.
- Nephrolithiasis (kidney stones).
Drug Interactions
Digoxin
May increase the risk of cardiac arrhythmias.
Levothyroxine
Calcium can interfere with levothyroxine absorption; administer at least 4 hours apart.
Bisphosphonates
Calcium can interfere with bisphosphonate absorption; administer at least 30 minutes apart.
Corticosteroids
May reduce calcium absorption.
Thiazide diuretics
May increase the risk of hypercalcemia.
Phenytoin/Barbiturates
May increase vitamin D metabolism.
Tetracyclines & Quinolone antibiotics
Calcium can reduce their absorption; administer at least 2-3 hours apart.
Storage
Store in a cool, dry place, below 30°C, away from direct sunlight. Keep out of reach of children.
Overdose
Symptoms include hypercalcemia (nausea, vomiting, constipation, thirst, polyuria, muscle weakness, fatigue). Treatment involves discontinuing the product, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Generally considered safe and often recommended during pregnancy and lactation for adequate calcium and vitamin D intake, but always consult a doctor.

Clinical Trials
Extensive clinical trials support the efficacy of calcium and vitamin D supplementation for bone health. Specific trials for Bonex-D would be proprietary.
Lab Monitoring
- Serum calcium levels (especially in patients with renal impairment or on prolonged therapy).
- Serum phosphate levels.
- Renal function (creatinine clearance).
- 25-hydroxyvitamin D levels (if vitamin D deficiency is being monitored).
Doctor Notes
- Advise patients to report any symptoms of hypercalcemia.
- Consider baseline and periodic monitoring of serum calcium, phosphate, and renal function.
- Ensure proper hydration.
Patient Guidelines
- Take as directed by your doctor.
- Do not exceed the recommended dose.
- Inform your doctor about all other medications you are taking.
- Maintain a balanced diet rich in calcium.
Missed Dose Advice
If a dose is missed, take it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
Bonex-D is not known to affect the ability to drive or operate machinery.
Lifestyle Advice
- Engage in weight-bearing exercises to improve bone strength.
- Ensure adequate sunlight exposure for natural vitamin D production.
- Avoid excessive alcohol and smoking, as they can negatively impact bone health.
