Description
Overview of the medicine
Brirel 60 mg Tablet contains Brirelol, an agent used in the treatment of anxiety disorders and major depressive disorder. It works by modulating serotonin neurotransmission in the brain.
Uses & Indications
Dosage
Adults
Initial dose 30 mg once daily, increased to 60 mg once daily after 1 week if tolerated. Maximum 120 mg/day.
Elderly
Initial dose 30 mg once daily. Dose adjustments should be made carefully based on renal function and tolerability.
Renal_impairment
No dose adjustment needed for mild to moderate impairment. For severe impairment (CrCl < 30 mL/min), initiate at 30 mg once daily and monitor closely.
How to Take
Take orally with or without food, preferably at the same time each day.
Mechanism of Action
Brirelol, a selective serotonin reuptake enhancer (SSRE), increases the presynaptic reuptake of serotonin in the brain, thereby enhancing serotonergic neurotransmission. This mechanism is distinct from SSRIs which inhibit reuptake.
Pharmacokinetics
Onset
Clinical effects may be observed within 1-2 weeks, with full therapeutic benefit achieved after 4-6 weeks.
Excretion
Mainly renal, with a small portion excreted in feces.
Half life
Approximately 12-18 hours.
Absorption
Rapidly absorbed orally, with peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily hepatic via CYP450 enzymes, predominantly CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to Brirelol or any excipients
- Concomitant use with MAOIs
- Severe hepatic impairment
Drug Interactions
MAOIs
Fatal interaction; allow at least 14 days between discontinuing an MAOI and starting Brirelol, and vice-versa.
Alcohol
May potentiate CNS depressant effects; avoid concomitant use.
CYP3A4 inhibitors
May increase Brirelol plasma concentrations, requiring dose adjustment.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, nausea, vomiting, dizziness, tachycardia, and tremor. Treatment is supportive and symptomatic; no specific antidote.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding due to potential excretion into breast milk.
