Description
Overview of the medicine
Brivaracetam is an antiepileptic drug (AED) used to treat partial-onset seizures with or without secondary generalization in patients 4 years of age and older. It is thought to exert its anticonvulsant effects by selectively binding to the synaptic vesicle protein 2A (SV2A) in the brain.
Uses & Indications
Dosage
Adults
Initial recommended dose is 50 mg twice daily. Depending on individual patient response and tolerability, the dose may be adjusted from 25 mg twice daily to 100 mg twice daily. Doses should be titrated gradually.
Elderly
No specific dosage adjustment is required based on age alone. However, caution is advised due to potential for decreased renal or hepatic function.
Renal_impairment
No dose adjustment is required for mild to moderate renal impairment. For severe renal impairment, consider a maximum initial dose of 50 mg twice daily, with careful monitoring.
How to Take
Brivaracetam tablets can be taken with or without food. Swallow the tablet whole with water; do not crush, chew, or split it.
Mechanism of Action
Brivaracetam exhibits high and selective affinity for synaptic vesicle protein 2A (SV2A) in the brain, which is thought to be the primary mechanism by which it exerts its antiepileptic effects. The exact physiological role of SV2A is not fully understood, but it is involved in regulating neurotransmitter release.

Onset
Within hours, but full therapeutic effect may take several days of consistent dosing.
Excretion
Approximately 95% of the dose is excreted in urine within 72 hours, with about 88% as metabolites and 8% as unchanged drug.
Half life
Approximately 9 hours.
Absorption
Rapid and complete absorption after oral administration, with peak plasma concentrations typically reached within 2 hours. Bioavailability is approximately 100%.
Metabolism
Primarily metabolized by hydrolysis of the amide moiety, mainly by hepatic CYP450 enzymes (CYP2C19) and non-CYP pathways. The main metabolite is inactive.
Side Effects
Contraindications
- •Hypersensitivity to brivaracetam or any other pyrrolidone derivatives, or to any of the excipients.
- •Severe hepatic impairment (use with caution, consider dose adjustment).
Drug Interactions
Rifampicin
May decrease brivaracetam plasma concentrations. Dose adjustment of brivaracetam may be necessary.
Oral Contraceptives
Brivaracetam at doses of 100 mg twice daily has been shown to reduce the plasma concentrations of ethinylestradiol and levonorgestrel. Patients should be advised to use additional or alternative non-hormonal contraception if necessary.
Phenytoin, Carbamazepine, Phenobarbital
May decrease brivaracetam levels; co-administration may require brivaracetam dose adjustment. Conversely, brivaracetam may slightly increase phenytoin levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include dizziness, somnolence, nausea, vomiting, and tremors. There is no specific antidote. Treatment should be supportive and symptomatic, including general supportive measures and monitoring of vital signs.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Brivaracetam is excreted into breast milk; therefore, a decision should be made whether to discontinue breastfeeding or to discontinue the drug, taking into account the importance of the drug to the mother.
