Overview of the medicine
Bukof-D 75 mg Syrup is a combination medication used to relieve symptoms of cough, nasal congestion, and sneezing associated with the common cold, allergies, or other respiratory conditions. It contains a cough suppressant (Dextromethorphan) and a decongestant (Phenylephrine).
Uses & Indications
Dosage
Adults
10-20 mL orally every 4-6 hours as needed, not to exceed 80 mL in 24 hours.
Elderly
Similar to adult dosage; however, use with caution and monitor for side effects, especially in patients with comorbidities.
Renal_impairment
Use with caution in patients with severe renal impairment; dose adjustment may be necessary. Consult a doctor.
How to Take
Administer orally using the provided measuring cup or spoon to ensure accurate dosing. Do not exceed the recommended dosage. Can be taken with or without food.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla to elevate the cough threshold, thereby reducing the urge to cough. Phenylephrine acts as an alpha-adrenergic agonist, causing vasoconstriction in the nasal passages to reduce swelling and congestion.

Onset
Onset of action typically occurs within 15-30 minutes for both components.
Excretion
Both active ingredients and their metabolites are primarily excreted via the kidneys in urine.
Half life
Dextromethorphan: approximately 2-4 hours (can be up to 12 hours in poor metabolizers). Phenylephrine: approximately 2-3 hours.
Absorption
Dextromethorphan and Phenylephrine are rapidly absorbed from the gastrointestinal tract after oral administration.
Metabolism
Dextromethorphan undergoes extensive hepatic metabolism, primarily by CYP2D6. Phenylephrine is metabolized in the gut wall and liver by monoamine oxidase (MAO).
Side Effects
Contraindications
- •Hypersensitivity to Dextromethorphan, Phenylephrine, or any other component of the syrup.
- •Patients currently taking monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy.
- •Severe hypertension or severe coronary artery disease.
- •Known severe hepatic or renal impairment.
- •Narrow-angle glaucoma, urinary retention, or prostatic hypertrophy.
Drug Interactions
Antihypertensive drugs
Phenylephrine may reduce the effectiveness of antihypertensive medications.
Monoamine Oxidase Inhibitors (MAOIs)
Concomitant use with MAOIs (or within 14 days of stopping) can lead to a hypertensive crisis, severe hyperpyrexia, and convulsions due to Phenylephrine.
Central Nervous System (CNS) Depressants
Concurrent use with alcohol, sedatives, hypnotics, or other CNS depressants may enhance drowsiness and respiratory depression.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children. Do not refrigerate.
Overdose
Symptoms of overdose may include severe drowsiness, dizziness, nausea, vomiting, blurred vision, ataxia, nystagmus, hallucinations, agitation, respiratory depression, and convulsions. In case of overdose, seek immediate medical attention. Treatment is symptomatic and supportive, including activated charcoal if ingestion was recent, and respiratory support if needed.
Pregnancy & Lactation
Pregnancy Category C. Studies in animals have shown adverse effects on the fetus, and there are no adequate and well-controlled studies in pregnant women. Use only if the potential benefit justifies the potential risk to the fetus. Consult a doctor. It is not recommended during lactation without medical advice, as active ingredients may pass into breast milk.
