Description
Overview of the medicine
Burnrid 1% Cream is a topical antibacterial preparation containing Silver Sulfadiazine. It is used to prevent and treat wound infections in patients with second- and third-degree burns.
Uses & Indications
Dosage
Adults
Apply a layer of cream, 1-2 mm thick, to the affected area once or twice daily. The cream should be reapplied to any areas where it has been removed by patient activity.
Elderly
No specific dose adjustments required for elderly patients.
Renal_impairment
Use with caution in patients with severe renal impairment due to potential systemic absorption of sulfadiazine. Monitor serum sulfadiazine levels if extensive absorption is suspected.
How to Take
For external use only. Clean the wound before applying the cream. Apply with a sterile glove or spatula to the affected area. Do not ingest.
Mechanism of Action
Silver Sulfadiazine acts on the cell membrane and cell wall of bacteria, leading to disruption of their structure and function. The silver ion disrupts bacterial DNA and RNA, while sulfadiazine inhibits folic acid synthesis, a vital component for bacterial growth.
Pharmacokinetics
Onset
Antibacterial effect begins rapidly after application.
Excretion
Systemically absorbed sulfadiazine and its metabolites are primarily excreted via the kidneys.
Half life
Not well-defined for topical application; systemically absorbed sulfadiazine has a half-life of 10-12 hours.
Absorption
Minimal systemic absorption; however, absorption of silver and sulfadiazine can occur, especially with extensive burn areas.
Metabolism
Systemically absorbed sulfadiazine is metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to silver sulfadiazine or other sulfonamides.
- Premature infants or neonates (risk of kernicterus).
- Pregnancy near term, particularly in the third trimester (risk of kernicterus in the fetus).
- Glucose-6-phosphate dehydrogenase deficiency (risk of hemolysis).
Drug Interactions
Cimetidine
Systemic cimetidine may increase the incidence of leukopenia associated with silver sulfadiazine.
Proteolytic enzymes
Should not be used concurrently with enzymatic debriding agents as silver may inactivate these enzymes.
Storage
Store below 30°C, away from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Systemic absorption and toxicity are unlikely with proper topical application. If large amounts are ingested or extensively absorbed, monitor for signs of systemic sulfonamide toxicity (e.g., nausea, vomiting, renal dysfunction, blood dyscrasias). Treatment is symptomatic and supportive.
Pregnancy & Lactation
Contraindicated near term in pregnancy. Use with caution during other trimesters if potential benefits outweigh risks. Avoid during lactation as sulfonamides are excreted in breast milk.

Lab Monitoring
- Complete blood count (CBC) if extensive application and prolonged use, to monitor for leukopenia.
- Renal function tests if systemic absorption is suspected or in patients with pre-existing renal impairment.
Doctor Notes
- Assess burn severity and extent before prescribing. Ensure adequate debridement.
- Monitor for signs of systemic toxicity, especially in patients with extensive burns, renal impairment, or G6PD deficiency.
- Advise patients on proper application technique and wound care.
Patient Guidelines
- Wash hands before and after applying the cream.
- Apply only to the affected skin area as directed by your doctor.
- Avoid contact with eyes, nose, and mouth.
- Do not cover the treated area with a bandage unless directed by a doctor.
Missed Dose Advice
