Overview of the medicine
Calcin-D 500 mg Tablet is a combination medicine containing Calcium Carbonate and Vitamin D3 (Colecalciferol). It is used to prevent and treat calcium and vitamin D deficiencies, which are essential for maintaining healthy bones and teeth.
Uses & Indications
Dosage
Adults
Usually 1-2 tablets daily, or as directed by the physician. Should be taken with a meal.
Elderly
Similar to adult dosage, but caution should be exercised in patients with renal impairment or a history of kidney stones. Dosage adjustment may be required based on serum calcium and vitamin D levels.
Renal_impairment
Use with caution. Dosage may need to be reduced. Regular monitoring of serum calcium and phosphate levels is essential. Contraindicated in severe renal impairment.
How to Take
Oral administration. Tablets should be swallowed whole with water, preferably with a meal to enhance absorption and reduce gastric upset.

Absorption
Calcium absorption from the GI tract is variable (20-30%) and occurs via active transport and passive diffusion, enhanced by Vitamin D3. Vitamin D3 is absorbed from the small intestine, primarily in the jejunum, and transported via chylomicrons.
Metabolism
Calcium is mainly deposited in bones. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D3 (calcifediol) and then in the kidneys to the active form, 1,25-dihydroxyvitamin D3 (calcitriol).

Contraindications
- Hypercalcemia (high levels of calcium in the blood)
- Hypercalciuria (high levels of calcium in the urine)
- Hypervitaminosis D (excessive levels of Vitamin D)

- Hypersensitivity to calcium carbonate, Vitamin D3, or any component of the formulation
Drug Interactions
Bisphosphonates
Calcium can interfere with bisphosphonate absorption; take at least 30 minutes to 1 hour apart.
Corticosteroids
Can decrease calcium absorption.
Thiazide Diuretics
May increase the risk of hypercalcemia by reducing calcium excretion.
Digitalis Glycosides
Hypercalcemia can potentiate the effects of digitalis, leading to cardiac arrhythmias.
Tetracyclines & Fluoroquinolones
Calcium can reduce the absorption of these antibiotics; take at least 2-3 hours apart.
Thyroid Hormones (Levothyroxine)
Calcium can reduce absorption of levothyroxine; take at least 4 hours apart.

Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, abdominal pain, muscle weakness, polyuria, polydipsia, lethargy, confusion, and in severe cases, cardiac arrhythmias and coma). Management involves discontinuing the product, hydration, and medical supervision. In severe cases, corticosteroids or calcitonin may be used.

Calcin-D is generally considered safe and often recommended during pregnancy and lactation to meet increased calcium and vitamin D requirements, but only under the guidance of a physician. Excessive doses should be avoided.
