Overview of the medicine
Calcitonin is a naturally occurring polypeptide hormone that plays a role in calcium and bone metabolism. Calcitonin salmon is a synthetic form used to treat certain bone conditions. It inhibits bone resorption and decreases blood calcium levels.
Uses & Indications
Dosage
Adults
Postmenopausal osteoporosis: 100 IU SC or IM daily or every other day. Paget's disease: 100 IU SC or IM daily or 50 IU twice daily. Hypercalcemia: Initial 4 IU/kg SC or IM every 12 hours, may increase to 8 IU/kg every 6-8 hours.
Elderly
Dosage adjustment usually not required, but monitor renal function.
Renal_impairment
No specific dosage adjustment for mild to moderate impairment. Use with caution in severe renal impairment.
How to Take
Administered via subcutaneous (SC) or intramuscular (IM) injection. For hypercalcemia, it can also be given intravenously in some cases, but SC/IM is more common. Patients should be taught proper self-injection techniques if administered at home.
Mechanism of Action
Calcitonin acts directly on osteoclasts (cells responsible for bone resorption) by binding to specific receptors, inhibiting their activity. This leads to decreased bone breakdown and subsequent reduction in calcium release from bones into the bloodstream. It also increases renal excretion of calcium and phosphate.

Mainly excreted by the kidneys as inactive metabolites.
Half life
Approximately 20-30 minutes (plasma half-life) to 60-90 minutes (elimination half-life) for calcitonin salmon.

Rapidly absorbed after subcutaneous (SC) or intramuscular (IM) injection. Peak plasma concentration achieved within 15-30 minutes. Bioavailability is ~70% for SC/IM.
Metabolism

Storage
Unopened vials: Store in a refrigerator at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light. Once opened: Store at room temperature (15°C to 30°C / 59°F to 86°F) for up to 28 days.
Overdose
Symptoms of overdose may include severe hypocalcemia with tetany and possibly nausea and vomiting. Management involves symptomatic and supportive care, including intravenous calcium if hypocalcemia is severe.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether calcitonin is excreted in human milk; caution should be exercised when administered to a nursing woman.
