Description
Overview of the medicine
Calcizen-300-mg-tablet is a calcium supplement used to prevent or treat low blood calcium levels in people who do not get enough calcium from their diets. It is essential for healthy bones, muscles, nervous system, and heart.
Uses & Indications
Dosage
Adults
1-2 tablets (300-600 mg elemental calcium) daily, preferably with meals, or as directed by a physician. When used as an antacid, 2-4 tablets as needed.
Elderly
Similar to adult dosage, but caution should be exercised and calcium levels monitored, especially in those with renal impairment.
Renal_impairment
Use with caution. Dosage should be adjusted based on serum calcium and phosphate levels. High doses may lead to hypercalcemia or exacerbate existing renal issues. Consult a physician.
How to Take
Take orally with a full glass of water. For best absorption, it is often recommended to take calcium carbonate with food, especially if used as a supplement. If used as an antacid, chew tablets thoroughly before swallowing or let them dissolve in the mouth.
Mechanism of Action
Calcium carbonate works by providing calcium ions, which are essential for many physiological processes including bone formation, nerve impulse transmission, muscle contraction, and blood coagulation. It neutralizes stomach acid, allowing for better absorption of calcium.
Pharmacokinetics
Onset
Onset of action for calcium supplementation is gradual, primarily reflecting changes in bone mineral density over weeks to months. Acute effects on serum calcium are relatively rapid.
Excretion
Excreted primarily via the kidneys (glomerular filtration and tubular reabsorption) and feces (unabsorbed calcium). Small amounts are also lost through sweat.
Half life
The physiological half-life of calcium in the body is complex and highly regulated, typically in the range of 5-7 days for bone turnover. Circulating calcium is tightly controlled.
Absorption
Calcium carbonate is absorbed in the small intestine, primarily through active transport and passive diffusion. Absorption is enhanced by Vitamin D and an acidic environment. Bioavailability can vary depending on food intake.
Metabolism
Calcium is not metabolized in the traditional sense; it functions as an ion. It is incorporated into bone and released from it as needed.
Side Effects
Contraindications
- •Hypercalcemia (high levels of calcium in the blood)
- •Severe hypercalciuria (high levels of calcium in the urine)
- •History of kidney stones (recurrent calcium oxalate stones)
- •Known hypersensitivity to calcium carbonate or any component of the formulation
Drug Interactions
Bisphosphonates
Calcium can interfere with the absorption of bisphosphonates. Administer at least 30 minutes to 2 hours apart.
Iron supplements
Calcium may decrease iron absorption. Take iron supplements a few hours apart from calcium.
Thiazide diuretics
May reduce calcium excretion, increasing the risk of hypercalcemia.
Tetracycline and Quinolone antibiotics
Calcium can bind to these antibiotics in the gastrointestinal tract, reducing their absorption and effectiveness. Administer at least 2-3 hours apart.
Thyroid hormones (e.g., Levothyroxine)
Reduced absorption of thyroid hormones. Separate administration by at least 4 hours.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe constipation, nausea, vomiting, unusual tiredness, muscle weakness, confusion, increased thirst, and frequent urination. In severe cases, it can lead to kidney damage and cardiac arrhythmias. Management involves discontinuing calcium supplements, ensuring adequate hydration, and symptomatic treatment. Consult a physician immediately.
Pregnancy & Lactation
Calcium carbonate is generally considered safe during pregnancy and lactation. Adequate calcium intake is crucial for both mother and developing baby. However, it should be used under medical supervision, and excessive doses should be avoided.
