Description
Overview of the medicine
Caldil 500 mg tablet is a dietary supplement containing Calcium Carbonate and Vitamin D3. It is primarily used to prevent and treat calcium deficiencies and maintain bone health, particularly in conditions like osteoporosis, rickets, and during pregnancy and lactation.
Uses & Indications
Dosage
Adults
Typically, 1 to 2 tablets daily, or as directed by physician. Should be taken with food for better absorption.
Elderly
Same as adult dosage, but caution with renal function. Consult physician.
Renal_impairment
Caution required. Dosage adjustment may be necessary based on serum calcium and phosphate levels. Avoid in severe renal impairment.
How to Take
Oral administration. Tablets should be swallowed whole with water, preferably with meals to enhance calcium absorption and minimize gastrointestinal upset.
Mechanism of Action
Calcium Carbonate provides elemental calcium which is crucial for bone formation, nerve transmission, muscle contraction, and blood clotting. Vitamin D3 (Cholecalciferol) enhances the absorption of calcium from the intestine and regulates calcium and phosphate levels in the body, promoting bone mineralization.
Pharmacokinetics
Onset
Onset of therapeutic effects for bone health is gradual, over weeks to months.
Excretion
Calcium is primarily excreted via kidneys (urine) and feces. Vitamin D metabolites are primarily excreted via bile into feces, with some renal excretion.
Half life
Calcium: Not a single half-life, complex kinetics. Vitamin D3: Elimination half-life of 25-hydroxyvitamin D is about 2-3 weeks, active form 1,25-dihydroxyvitamin D is a few hours.
Absorption
Calcium carbonate is absorbed in the small intestine, with efficiency dependent on gastric acidity and vitamin D levels. Vitamin D3 is absorbed from the small intestine, typically with fatty foods, and transported to the liver and kidneys for activation.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D3 (calcifediol), then in the kidneys to the active form 1,25-dihydroxyvitamin D3 (calcitriol).
Side Effects
Contraindications
- Hypersensitivity to calcium carbonate or vitamin D3
- Hypercalcemia (high calcium levels in blood)
- Hypercalciuria (high calcium levels in urine)
- Severe renal impairment
- Nephrolithiasis (kidney stones)
- Hypervitaminosis D
Drug Interactions
Bisphosphonates
Reduce absorption; take calcium at least 30 minutes to 2 hours apart.
Thiazide diuretics
Increase risk of hypercalcemia.
Tetracycline antibiotics
Reduce absorption; take calcium 2-4 hours apart.
Fluoroquinolone antibiotics
Reduce absorption; take calcium 2-4 hours apart.
Cardiac glycosides (e.g., digoxin)
Increased risk of arrhythmias with hypercalcemia.
Thyroid hormones (e.g., levothyroxine)
Reduce absorption; take calcium 4 hours apart.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose (hypercalcemia) include nausea, vomiting, constipation, abdominal pain, muscle weakness, polyuria, polydipsia, and in severe cases, cardiac arrhythmias and coma. Treatment involves discontinuation of the supplement, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Caldil is generally considered safe and beneficial during pregnancy and lactation for meeting increased calcium and vitamin D requirements, when taken at recommended doses. Consult a doctor before use.
