Description
Overview of the medicine
Calfix-D is a nutritional supplement containing Calcium Carbonate and Vitamin D3 (Cholecalciferol), primarily used to support bone health and prevent/treat deficiencies of calcium and vitamin D.
Uses & Indications
Dosage
Adults
Typically 1-2 tablets daily, or as directed by a physician. Best taken with food.
Elderly
Similar to adults, but dosage may need adjustment based on individual calcium and vitamin D status and renal function. Consult a physician.
Renal_impairment
Use with caution. Regular monitoring of serum calcium and phosphate levels is recommended. Dosage adjustment may be necessary under medical supervision.
How to Take
Take the tablet orally, preferably with a meal to enhance absorption. Swallow the tablet whole with water; do not chew or crush.
Mechanism of Action
Calcium is an essential mineral required for bone formation, muscle contraction, nerve transmission, and various cellular functions. Vitamin D3 facilitates the absorption of calcium from the gut, regulates calcium and phosphate levels in the body, and promotes bone mineralization.
Pharmacokinetics
Onset
Acute effects on serum calcium can be seen within hours. Long-term benefits on bone density are observed over months.
Excretion
Calcium: Primarily excreted via feces (unabsorbed calcium), with a small amount excreted renally. Vitamin D3: Metabolites are primarily excreted in bile and feces, with some renal excretion.
Half life
Elemental Calcium: Complex pharmacokinetics, not typically assigned a single half-life. Vitamin D3 (Cholecalciferol): Elimination half-life is approximately 15-20 days, but active metabolites have longer half-lives.
Absorption
Calcium: Absorbed primarily in the small intestine; absorption efficiency varies (20-40%) and is enhanced by Vitamin D. Vitamin D3: Readily absorbed from the small intestine (60-90%) with fatty meals, primarily via the lymphatic system.
Metabolism
Calcium: Not metabolized in the traditional sense, but regulated by hormones. Vitamin D3: Hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol), then in the kidneys to the active form, 1,25-dihydroxyvitamin D (calcitriol).
Side Effects
Contraindications
- •Hypersensitivity to the active substances or to any of the excipients
- •Hypercalcemia (abnormally high levels of calcium in the blood)
- •Hypercalciuria (abnormally high levels of calcium in the urine)
- •Severe renal impairment or kidney stones
- •Primary hyperparathyroidism or other conditions leading to hypercalcemia
Drug Interactions
Digoxin
Hypercalcemia, when caused by high doses of vitamin D, may increase the risk of cardiac arrhythmias in patients taking digoxin.
Levothyroxine
Calcium can reduce the absorption of thyroid hormones. Separate administration by at least 4 hours.
Bisphosphonates
Calcium can interfere with the absorption of bisphosphonates. Administer calcium at a different time of day.
Thiazide Diuretics
May increase the risk of hypercalcemia by decreasing urinary calcium excretion.
Tetracycline and Fluoroquinolone Antibiotics
Calcium can chelate these antibiotics, reducing their absorption. Take calcium supplements at least 2-3 hours before or after these antibiotics.
Storage
Store below 30°C in a cool, dry place. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, thirst, polyuria, muscle weakness, confusion, and in severe cases, cardiac arrhythmias and coma). Management involves discontinuing the supplement, rehydration, and in severe cases, administration of loop diuretics or corticosteroids under medical supervision.
Pregnancy & Lactation
Calfix-D is generally considered safe and beneficial during pregnancy and lactation, as calcium and vitamin D are essential for maternal and fetal/infant health. However, use should be under medical supervision to avoid excessive dosing.
