Description
Overview of the medicine
Caloplax 2 mg tablet is an antiplatelet medication used to prevent blood clots in patients with cardiovascular diseases.
Uses & Indications
Dosage
Adults
Standard dose is 2 mg once daily. For acute coronary syndrome, a loading dose of 8 mg may be given, followed by 2 mg once daily.
Elderly
No dosage adjustment is generally required for elderly patients. However, caution is advised due to increased risk of bleeding.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment. For severe renal impairment, use with caution due to limited data.
How to Take
Caloplax 2 mg tablet should be taken orally, with or without food. It is recommended to take the tablet at the same time each day.
Mechanism of Action
Caloplax selectively inhibits the binding of adenosine diphosphate (ADP) to its P2Y12 receptor on platelets, thereby preventing activation of the GPIIb/IIIa complex and subsequent platelet aggregation. This action irreversibly modifies the platelet ADP receptor.
Pharmacokinetics
Onset
Platelet inhibition starts within 2 hours; full effect observed after 3-7 days of regular dosing.
Excretion
Approximately 50% excreted in urine and 48% in feces over 120 hours.
Half life
The active metabolite has an elimination half-life of approximately 8-12 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Bioavailability is approximately 30-40%.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (primarily CYP2C19 and CYP3A4) to an active metabolite and inactive metabolites.
Side Effects
Contraindications
- Active pathological bleeding (e.g., peptic ulcer, intracranial hemorrhage).
- Hypersensitivity to the active substance or to any of the excipients.
- Severe hepatic impairment.
Drug Interactions
NSAIDs (e.g., ibuprofen, naproxen)
Increased risk of gastrointestinal bleeding. Use with caution.
Warfarin and other oral anticoagulants
Increased risk of bleeding. Co-administration is not recommended.
Other antiplatelet agents (e.g., aspirin)
Increased risk of bleeding. Co-administration requires careful monitoring.
CYP2C19 inhibitors (e.g., omeprazole, fluoxetine)
May reduce the antiplatelet effect of Caloplax. Avoid concomitant use if possible.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose with Caloplax may lead to prolonged bleeding time and subsequent bleeding complications. Management involves supportive care and symptomatic treatment. If severe bleeding occurs, platelet transfusion may be considered, although there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, use during pregnancy should only be considered if the potential benefit justifies the potential risk. It is not known whether Caloplax is excreted in human milk; therefore, caution should be exercised when administered to a nursing mother.
