Description
Overview of the medicine
Caloren 1 mcg injection contains a synthetic active form of Vitamin D, often used to regulate calcium and phosphate metabolism. It is prescribed for conditions like hypoparathyroidism, renal osteodystrophy, and certain types of rickets.
Uses & Indications
Dosage
Adults
Initial dose 0.5-1 mcg intravenously or intramuscularly three times weekly, adjusted based on serum calcium levels. For hypoparathyroidism, 0.5-2 mcg daily.
Elderly
Similar to adult dosage, but with careful monitoring due to potential for increased sensitivity and co-morbidities.
Renal_impairment
Adjust dosage carefully; monitoring of calcium and phosphate levels is crucial. Often used specifically in renal impairment.
How to Take
Administer intravenously or intramuscularly as directed by a healthcare professional. Do not exceed prescribed dose. Ensure proper sterile technique.
Mechanism of Action
Caloren is the active form of Vitamin D3. It increases intestinal absorption of calcium and phosphate, promotes renal tubular reabsorption of calcium, and regulates bone mineralization. It also suppresses parathyroid hormone (PTH) secretion.
Pharmacokinetics
Onset
Within 2-4 hours, peak effect on serum calcium within 2-4 days.
Excretion
Eliminated through bile and feces.
Half life
Terminal half-life is approximately 5-8 hours, but biological effects may persist for several days.
Absorption
Rapidly absorbed after intravenous administration, distributed to target tissues.
Metabolism
Metabolized primarily in the liver by CYP24A1 (24-hydroxylase) into inactive metabolites.
Side Effects
Contraindications
- Hypercalcemia
- Vitamin D toxicity
- Hypersensitivity to Caloren or any of its components
- Evidence of metastatic calcification
Drug Interactions
Thiazide diuretics
May increase the risk of hypercalcemia.
Digitalis (digoxin)
Hypercalcemia can potentiate the effects of digitalis, leading to cardiac arrhythmias.
Phenobarbital, Phenytoin
May reduce Caloren's plasma levels due to increased metabolism.
Magnesium-containing antacids
May lead to hypermagnesemia, especially in patients with chronic renal dialysis.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, polyuria, polydipsia, weakness, headache). Treatment involves immediate discontinuation of Caloren, hydration, and correction of electrolyte imbalances. Furosemide or corticosteroids may be used in severe cases.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if Caloren is excreted in human milk; use with caution in nursing mothers.

- Renal function tests (creatinine, BUN)
Doctor Notes
- Emphasize strict adherence to calcium and vitamin D monitoring protocols.
- Educate patients on symptoms of hypercalcemia and the importance of reporting them immediately.
- Consider concurrent use of phosphate binders for patients on dialysis with hyperphosphatemia.

- Report any signs of hypercalcemia (e.g., nausea, vomiting, unusual thirst) to your doctor immediately.
- Do not take other vitamin D supplements or calcium supplements without consulting your doctor.
- Maintain adequate hydration as advised by your physician.
- Regular blood tests are essential to monitor treatment effectiveness and safety.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double dose.
