Description
Overview of the medicine
Camlodin Plus 5 mg tablet is a combination medication containing Amlodipine, a dihydropyridine calcium channel blocker, and Losartan potassium, an angiotensin II receptor blocker. It is primarily used to treat high blood pressure (hypertension). Amlodipine relaxes blood vessels, allowing blood to flow more easily, while Losartan blocks substances that narrow blood vessels, thereby lowering blood pressure.
Uses & Indications
Dosage
Adults
Typically one tablet once daily. The dose can be adjusted based on clinical response and tolerability, but usually, this combination is a fixed-dose.
Elderly
Initial dose adjustment may be considered, but generally same as adults, with careful monitoring.
Renal_impairment
No initial dose adjustment usually required for mild to moderate impairment, but careful monitoring of renal function and potassium levels is essential. Not recommended for severe renal impairment.
How to Take
Oral, usually once daily, with or without food.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle, causing peripheral arterial vasodilation and reducing afterload. Losartan potassium selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues (e.g., vascular smooth muscle, adrenal gland), which results in a decrease in vasoconstriction, aldosterone secretion, and sodium retention.

Onset
Amlodipine: Gradual onset of action (2-8 hours). Losartan: Significant reduction in BP within 6 hours.
Excretion
Amlodipine: Primarily renal (60% as metabolites, 10% unchanged). Losartan: Biliary and renal (approx 35% renal, 60% faecal).
Half life
Amlodipine: 30-50 hours. Losartan: 2 hours (Losartan) and 6-9 hours (active metabolite).
Absorption
Amlodipine: Well absorbed orally, bioavailability 64-90%. Peak plasma concentration 6-12 hours. Losartan: Well absorbed orally, bioavailability approx 33%. Undergoes significant first-pass metabolism. Peak plasma concentration 1 hour (Losartan) and 3-4 hours (active metabolite).
Metabolism
Amlodipine: Extensive hepatic metabolism to inactive metabolites. Losartan: Hepatic, primarily by CYP2C9 and CYP3A4, to an active carboxylic acid metabolite (E-3174).
Side Effects
Contraindications
- Hypersensitivity to Amlodipine, Losartan, or any excipients.
- Pregnancy (especially second and third trimesters).
- Severe hepatic impairment.
- Concomitant use with aliskiren in diabetic patients.
Drug Interactions
With Losartan
Potassium-sparing diuretics (e.g., spironolactone), potassium supplements, or salt substitutes containing potassium may increase serum potassium. NSAIDs may reduce the antihypertensive effect of losartan and increase the risk of renal impairment. Lithium levels may increase.
With Amlodipine
CYP3A4 inhibitors (e.g., ketoconazole, erythromycin) may increase amlodipine exposure. CYP3A4 inducers (e.g., rifampicin) may decrease amlodipine exposure. Simvastatin exposure may increase.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive hypotension (low blood pressure) and possibly bradycardia. Management is supportive, including blood pressure monitoring and intravenous fluids if needed. Activated charcoal may be considered soon after ingestion.
Pregnancy & Lactation
Contraindicated during pregnancy (especially 2nd and 3rd trimesters) due to fetal toxicity risk from Losartan. Not recommended during lactation as it is unknown if components are excreted in human milk.
