Description
Overview of the medicine
Candinil 150 mg Capsule contains Fluconazole, an antifungal medication used to treat various fungal infections, particularly those caused by Candida.
Uses & Indications
Dosage
Adults
For vaginal candidiasis: 150 mg as a single oral dose. For other indications, dosage varies, typically 50-400 mg daily.
Elderly
No specific dose adjustment for elderly patients with normal renal function. Dose adjustment may be needed based on renal function.
Renal_impairment
Dose adjustment required based on creatinine clearance. For CrCl < 50 mL/min, dosage should be reduced by 50%.
How to Take
Take orally, with or without food. Swallow the capsule whole with water.
Mechanism of Action
Fluconazole inhibits fungal cytochrome P450-dependent 14-alpha-demethylase, which is essential for ergosterol synthesis in fungal cell membranes, leading to impaired cell growth and viability.
Pharmacokinetics
Onset
Clinical effects may be observed within 24-72 hours, depending on the infection.
Excretion
Primarily renal, with approximately 80% of the dose excreted unchanged in urine.
Half life
Approximately 30 hours (range 20-50 hours), prolonged in renal impairment.
Absorption
Well absorbed orally, bioavailability >90%. Peak plasma concentrations achieved within 1-2 hours.
Metabolism
Minimal hepatic metabolism (approximately 11% is metabolized).
Side Effects
Contraindications
- Hypersensitivity to fluconazole or other azole antifungals.
- Co-administration with drugs known to prolong QT interval and metabolized by CYP3A4 (e.g., terfenadine, cisapride, astemizole, erythromycin).
Drug Interactions
Warfarin
Increased prothrombin time (INR) and risk of bleeding.
Phenytoin
Increased plasma concentrations of phenytoin.
Hydrochlorothiazide
Increased plasma concentrations of fluconazole.
Benzodiazepines (e.g., midazolam)
Increased plasma concentrations of benzodiazepines.
Oral Hypoglycemics (sulfonylureas)
Increased risk of hypoglycemia.
Cyclosporine, Tacrolimus, Sirolimus
Increased plasma concentrations of these immunosuppressants.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include hallucinations and paranoid behavior. Management involves symptomatic and supportive treatment. Hemodialysis can significantly reduce plasma concentrations.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Fluconazole is excreted in breast milk; caution is advised during breastfeeding.

Lab Monitoring
- Liver function tests (LFTs) for patients on prolonged therapy or with pre-existing hepatic impairment.
- Renal function monitoring in patients with renal impairment.
- Electrolyte monitoring (especially potassium, magnesium, calcium) if there's a risk of QT prolongation.
Doctor Notes
- Assess liver function before initiating and periodically during prolonged therapy.
- Be aware of significant drug interactions, especially with narrow therapeutic index drugs (e.g., warfarin, phenytoin, cyclosporine).
- Advise patients on potential for dizziness and QT prolongation risks.
- Ensure patient completes the full course to prevent recurrence and resistance.
Patient Guidelines
- Complete the full course of medication as prescribed by your doctor, even if symptoms improve.
- Inform your doctor about all other medications, including over-the-counter drugs and herbal supplements.
- Report any severe or persistent side effects, especially signs of liver problems (e.g., unusual tiredness, yellowing of skin/eyes, dark urine).
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and take your next dose at the regular time. Do not double the dose to catch up.
