Overview of the medicine
Captabine 150 mg Tablet is used to treat major depressive disorder, generalized anxiety disorder, and panic disorder by selectively inhibiting the reuptake of serotonin in the brain.
Uses & Indications
Dosage
Adults
Initial dose 100 mg once daily, may be increased to 150 mg once daily after one week based on response. Maximum recommended dose is 200 mg daily.
Elderly
Start with a lower dose (e.g., 50 mg daily) and titrate carefully due to potential increased sensitivity and reduced clearance.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Use with caution in severe impairment; consider lower doses.
How to Take
Take orally, once daily, with or without food. Swallow the tablet whole; do not crush, chew, or break.
Mechanism of Action
Captabine works by selectively inhibiting the reuptake of serotonin (5-HT) into presynaptic neurons in the central nervous system, thereby enhancing serotonergic neurotransmission.
Pharmacokinetics
Onset
Antidepressant effects may take 2-4 weeks to become fully apparent; anxiolytic effects may appear sooner.
Excretion
Primarily excreted renally as inactive metabolites.
Half life
Approximately 20-24 hours.
Absorption
Well absorbed orally, peak plasma concentrations reached in 4-8 hours.
Metabolism
Extensively metabolized in the liver primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to Captabine or any excipients
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy
- •Concomitant use with pimozide
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability).
Triptans
Possible serotonin syndrome.
Warfarin
Potentiation of anticoagulant effect.
NSAIDs/Aspirin
Increased risk of bleeding due to effects on platelet aggregation.
Other serotonergic drugs (e.g., tramadol, other SSRIs)
Increased risk of serotonin syndrome.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms may include somnolence, nausea, vomiting, tachycardia, tremor, agitation, and dizziness. Management is primarily supportive; maintain airway, monitor cardiac and vital signs. Activated charcoal may be considered. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Captabine is excreted in human milk; use with caution in nursing mothers.
