Description
Overview of the medicine
Carbiroid-5 mg Tablet is a selective cyclooxygenase-2 (COX-2) inhibitor used for the symptomatic relief of pain and inflammation in various conditions. It works by reducing the production of prostaglandins, which are responsible for pain and inflammation.
Uses & Indications
Dosage
Adults
Osteoarthritis: 5 mg once daily. Rheumatoid Arthritis: 5 mg once daily. Acute Pain/Dysmenorrhea: 5 mg as needed, not exceeding 10 mg per day.
Elderly
Initiate with lowest effective dose, 5 mg once daily, and monitor for side effects.
Renal_impairment
Mild to moderate impairment: No dosage adjustment required. Severe impairment: Use with caution, consider reduced dose or avoid.
How to Take
Orally, with or without food. Taking it with food may help reduce gastrointestinal upset. Swallow the tablet whole with a glass of water.
Mechanism of Action
Carbiroid selectively inhibits cyclooxygenase-2 (COX-2) enzyme, which is primarily responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. By inhibiting COX-2, Carbiroid reduces the inflammatory response without significantly affecting COX-1, which is involved in gastric protection and platelet function, thereby potentially offering a better gastrointestinal safety profile compared to non-selective NSAIDs.
Pharmacokinetics
Onset
Pain relief typically begins within 30-60 minutes; anti-inflammatory effects may take several days to become fully apparent.
Excretion
Mainly excreted in the urine (approx. 27%) and feces (approx. 57%) as metabolites.
Half life
Approximately 8-10 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (CYP2C9 and CYP3A4).
Side Effects
Contraindications
- Known hypersensitivity to Carbiroid or any component of the formulation
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Severe heart failure
- Active gastrointestinal bleeding or ulceration
- Third trimester of pregnancy
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Warfarin
Increased risk of bleeding.
Diuretics
Reduced diuretic effect; increased risk of renal impairment.
SSRIs/SNRIs
Increased risk of gastrointestinal bleeding.
Methotrexate
Increased methotrexate toxicity.
ACE Inhibitors/ARBs
Reduced antihypertensive effect; increased risk of renal impairment.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Rare cases of hypertension, acute renal failure, respiratory depression, and coma have occurred. Management is primarily supportive. Induce vomiting and/or administer activated charcoal if ingestion is recent. Monitor renal function and blood pressure.
Pregnancy & Lactation
Pregnancy: Use of NSAIDs, including Carbiroid, in the third trimester of pregnancy increases the risk of premature closure of the fetal ductus arteriosus. Avoid during late pregnancy. Use during early pregnancy only if the potential benefit outweighs the potential risk to the fetus. Lactation: It is unknown whether Carbiroid is excreted in human milk. Weigh the benefits of breastfeeding against the potential risks to the infant.

Lab Monitoring
- Monitor blood pressure regularly, especially in patients with hypertension.
- Monitor renal function (creatinine, BUN) periodically, especially in elderly patients or those with pre-existing renal impairment.
- Monitor liver function tests (ALT, AST) periodically.
- Monitor for signs of gastrointestinal bleeding (e.g., CBC for anemia).
Doctor Notes
- Prescribe the lowest effective dose for the shortest duration consistent with individual patient treatment goals.
- Periodically reassess the need for Carbiroid in chronic pain management.
- Consider gastroprotective agents for patients at high risk of GI complications.
Patient Guidelines
- Take Carbiroid exactly as prescribed by your doctor.
- Do not exceed the recommended dose.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
- Report any signs of unusual bleeding, dark stools, or severe stomach pain immediately.
- Be aware of the potential cardiovascular risks associated with NSAIDs.
