Description
Overview of the medicine
Carbofer 500 mg Injection contains Ferric Carboxymaltose, an iron replacement product used to treat iron deficiency anemia when oral iron is ineffective or cannot be used.
Uses & Indications
Dosage
Adults
The total cumulative dose is determined based on body weight and hemoglobin level. Typically, single doses of up to 1000 mg (or 15 mg/kg) iron can be administered intravenously, repeated at least 7 days apart if needed, up to a maximum cumulative dose.
Elderly
No specific dose adjustment is generally required, but caution should be exercised due to potential comorbidities.
Renal_impairment
No dose adjustment is usually needed for patients with non-dialysis dependent CKD or dialysis-dependent CKD.
How to Take
Carbofer 500 mg Injection is for intravenous administration only. It should be given as a slow intravenous push or diluted in up to 250 mL of normal saline and infused over at least 15 minutes. It should be administered by a healthcare professional.
Mechanism of Action
Ferric Carboxymaltose is a non-dextran iron complex that allows for controlled release of iron to the body's iron stores, primarily in the bone marrow for hemoglobin synthesis and also into ferritin, bypassing the gastrointestinal tract.
Pharmacokinetics
Onset
Rapid increase in serum iron, but full erythropoietic response and increase in hemoglobin take several weeks.
Excretion
Minimal iron excretion. The iron is primarily used for hemoglobin and stored as ferritin, with very little eliminated via kidneys or feces.
Half life
Total iron has a half-life of 7-12 hours; the iron complex itself is cleared from plasma within 24 hours.
Absorption
Administered intravenously, absorption is bypassed. Iron is released slowly from the complex.
Metabolism
The complex is taken up by the reticuloendothelial system, where iron is released and then enters the physiological iron stores.
Side Effects
Contraindications
- Hypersensitivity to Ferric Carboxymaltose or any of its excipients
- Anemia not attributed to iron deficiency (e.g., iron overload)
- Evidence of iron overload or disturbances in iron utilization
Drug Interactions
Oral Iron Preparations
Concomitant administration of oral iron preparations may reduce the absorption of oral iron and increase the risk of iron overload. Should be withheld for at least 5 days after the last Ferric Carboxymaltose injection.
Other Parenteral Iron Preparations
Should not be administered concurrently as it may lead to iron overload.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of reach of children.
Overdose
Overdose may lead to iron overload (hemosiderosis), which can be managed with iron chelating agents (e.g., deferoxamine) if necessary. Symptoms may include gastrointestinal distress, cardiovascular collapse, and liver damage.
Pregnancy & Lactation
Pregnancy Category C (US FDA, based on older system, newer system advises weighing risks/benefits). Use only if the potential benefit justifies the potential risk to the fetus. Small amounts of iron may pass into breast milk, but generally considered safe for breastfed infants. Consult with a doctor.

Clinical Trials
Ferric Carboxymaltose has been evaluated in numerous clinical trials demonstrating its efficacy and safety in treating iron deficiency anemia across various patient populations, including those with chronic kidney disease, inflammatory bowel disease, and postpartum iron deficiency.
Lab Monitoring
- Hemoglobin and hematocrit levels (before treatment and periodically after)
- Serum ferritin and transferrin saturation (to assess iron status)
- Serum phosphate levels (to monitor for hypophosphatemia, especially with multiple doses)
Doctor Notes
- Strictly adhere to administration guidelines, including slow infusion rate and post-infusion observation for hypersensitivity reactions.
- Monitor hemoglobin, ferritin, and transferrin saturation to guide treatment decisions and prevent iron overload.
- Be aware of the risk of symptomatic hypophosphatemia and monitor serum phosphate levels when appropriate.
Patient Guidelines
- This medicine should only be administered by a healthcare professional in a clinical setting.
- Report any unusual or severe reactions (e.g., rash, swelling, difficulty breathing) during or after the infusion immediately.
- Do not take any oral iron supplements unless advised by your doctor.
Missed Dose Advice
If a dose is missed, contact your doctor or healthcare provider immediately to reschedule your next dose. Do not attempt to take a double dose.
Driving Precautions
May cause dizziness or lightheadedness. Patients should be advised to observe how they react to Carbofer 500 mg Injection before driving or operating machinery.
Lifestyle Advice
