Description
Overview of the medicine
Activated charcoal is a highly adsorbent material used to treat acute oral poisoning and drug overdose by binding toxins in the gastrointestinal tract, preventing their absorption. It can also be used for relief from gas and bloating.
Uses & Indications
Dosage
Adults
For gas/bloating: 375 mg 2 to 4 times daily, or as directed by a physician. For acute poisoning, much higher doses (25-100 g) are typically used under medical supervision.
Elderly
Same as adults, but caution advised due to potential for constipation. Consult a physician.
Renal_impairment
No specific dosage adjustment needed as it is not systemically absorbed.
How to Take
Take orally with a full glass of water. For poisoning, it may be given as a slurry. If taking for gas/bloating, it can be taken after meals.
Mechanism of Action
Activated charcoal adsorbs various substances by physical binding, forming a complex that is then excreted in the feces, thus preventing systemic absorption. It does not get absorbed into the bloodstream itself.
Pharmacokinetics
Onset
Rapid (within minutes to hours) once it reaches the gastrointestinal tract.
Excretion
Excreted unchanged in feces, along with the adsorbed substances.
Half life
Not applicable as it is not absorbed into the systemic circulation.
Absorption
Not absorbed systemically from the gastrointestinal tract.
Metabolism
Not metabolized by the body.
Side Effects
Contraindications
- Unprotected airway (risk of aspiration)
- Ingestion of caustic agents (acids/alkalis), petroleum distillates, or heavy metals (charcoal is ineffective and can obscure endoscopy)
- Intestinal obstruction or ileus
- Recent gastrointestinal surgery
Drug Interactions
Oral contraceptives
May reduce efficacy of birth control pills due to adsorption. Use alternative contraception.
Other oral medications
Activated charcoal can adsorb most oral medications, significantly reducing their absorption. Administer at least 2 hours before or after other oral drugs.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Activated charcoal is not typically toxic in overdose itself, but excessive amounts without sufficient fluid can lead to severe constipation, fecal impaction, or intestinal obstruction. Management is supportive, focusing on hydration and bowel management.
Pregnancy & Lactation
Generally considered safe during pregnancy and lactation as it is not systemically absorbed. However, always consult a healthcare professional before use.
