Overview of the medicine
Carbotor is an oral antidiabetic agent used for the management of type 2 diabetes mellitus. It belongs to a new class of drugs that improve glucose homeostasis by enhancing glucose-dependent insulin secretion and improving insulin sensitivity.
Uses & Indications
Dosage
Adults
Initial dose: 250 mg once daily, taken with the first main meal. May be increased to 500 mg once daily based on glycemic control and tolerability.
Elderly
No specific dose adjustment is required, but caution is advised due to potential renal function decline. Start with the lowest effective dose.
Renal_impairment
For mild to moderate renal impairment (CrCl 30-80 mL/min), no dose adjustment is generally needed. For severe renal impairment (CrCl <30 mL/min), Carbotor is not recommended.
How to Take
Take Carbotor orally, once daily with a main meal to minimize gastrointestinal side effects. Swallow the tablet whole; do not crush, chew, or break it.
Mechanism of Action
Carbotor selectively activates glucokinase, an enzyme crucial for glucose metabolism. By doing so, it increases the sensitivity of pancreatic beta-cells to glucose, leading to enhanced glucose-dependent insulin secretion. It also promotes glucose uptake and utilization in peripheral tissues and reduces hepatic glucose production.

Onset
Glycemic lowering effect typically observed within 30-60 minutes post-dose.
Excretion
Excreted mainly through urine (70% as metabolites) and feces (20%).
Half life
Elimination half-life is approximately 8-10 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations achieved within 1-2 hours. Bioavailability is approximately 70-80%.
Metabolism
Primarily metabolized in the liver by CYP3A4 and glucuronidation pathways into inactive metabolites.
Side Effects
Contraindications

Drug Interactions
Beta-blockers
May mask symptoms of hypoglycemia.
Corticosteroids
May increase blood glucose levels, requiring dose adjustment of Carbotor.
Diuretics (Thiazide)
May reduce the glucose-lowering effect of Carbotor.
Insulin and Sulfonylureas
Increased risk of hypoglycemia when used concurrently; careful monitoring and dose adjustment are necessary.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe hypoglycemia, nausea, vomiting, and dizziness. In case of overdose, symptomatic and supportive measures should be initiated. Oral glucose or intravenous glucose may be administered for hypoglycemia. Gastric lavage and activated charcoal may be considered if ingestion is recent.
Pregnancy & Lactation
Limited data on Carbotor use in pregnancy. Should be used only if the potential benefit justifies the potential risk to the fetus. It is not known whether Carbotor is excreted in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
