Description
Overview of the medicine
Clopidogrel is an antiplatelet medication used to prevent blood clots in patients with cardiovascular and cerebrovascular diseases.
Uses & Indications
Dosage
Adults
75 mg once daily. For ACS (without ST elevation) or MI with ST elevation, a loading dose (e.g., 300 mg or 600 mg) may be given, followed by 75 mg daily.
Elderly
No dose adjustment generally required, but caution with bleeding risk.
Renal_impairment
No dose adjustment for mild to moderate impairment; caution in severe impairment.
How to Take
Administer orally, once daily, with or without food.
Mechanism of Action
Clopidogrel is a prodrug that acts by irreversibly inhibiting the P2Y12 ADP chemoreceptor on platelets. This prevents activation of the glycoprotein GPIIb/IIIa complex, thereby inhibiting platelet aggregation.
Pharmacokinetics
Onset
Antiplatelet effect seen within 2 hours; maximum effect within 3-7 days.
Excretion
Approximately 50% renally, 48% fecally within 120 hours.
Half life
Parent compound: ~8 hours. Active metabolite: ~30 minutes (but irreversible action means effect lasts for platelet lifespan, ~7-10 days).
Absorption
Rapidly absorbed after oral administration. Bioavailability of the active metabolite is about 30-50%.
Metabolism
Primarily hepatic (CYP2C19, CYP1A2, CYP2B6, CYP3A4). Forms active metabolite and inactive carboxylic acid derivative.
Side Effects
Contraindications
- Hypersensitivity to clopidogrel or any component.
- Active pathological bleeding (e.g., peptic ulcer, intracranial hemorrhage).
- Severe hepatic impairment.
Drug Interactions
Warfarin, NSAIDs, other antiplatelets (e.g., Aspirin)
Increased risk of bleeding.
Proton Pump Inhibitors (PPIs) (e.g., Omeprazole, Esomeprazole)
May reduce active metabolite formation, diminishing antiplatelet effect. Avoid concomitant use if possible.
Storage
Store below 30°C in a dry place, away from direct light and moisture.
Overdose
Overdose may lead to increased bleeding complications. Management is supportive, and platelet transfusions may be considered in cases of severe bleeding.
Pregnancy & Lactation
Pregnancy Category B. Should be used during pregnancy only if clearly needed. It is unknown whether clopidogrel is excreted in human milk; a decision should be made whether to discontinue nursing or the drug.

YesClinical Trials
Extensive clinical trials (e.g., CAPRIE, CURE, COMMIT, CREDO, PCI-CURE) have established its efficacy in preventing atherothrombotic events in various patient populations.
Lab Monitoring
- Platelet function tests (if effectiveness concerns)
- Hemoglobin/Hematocrit (for bleeding monitoring)
Doctor Notes
- Assess bleeding risk factors (e.g., age, concomitant medications, history of GI bleeding) before initiating clopidogrel.
- Consider CYP2C19 genetic testing in patients at high risk of treatment failure or adverse events, especially if alternative antiplatelet therapy is being considered.
Patient Guidelines
- Take as directed, do not stop without doctor's advice.
- Report any unusual bleeding or bruising to your doctor immediately.
- Inform all healthcare providers, including dentists, about clopidogrel before any surgery or dental procedure.
Missed Dose Advice
If less than 12 hours since missed dose, take immediately. If more than 12 hours, skip and take next dose at regular time. Do not double dose.
Driving Precautions
Clopidogrel generally does not affect the ability to drive or use machines. However, if side effects such as dizziness occur, driving should be avoided.
Lifestyle Advice
- Avoid activities with high risk of injury.
- Maintain good oral hygiene to minimize gum bleeding.
