Cardisan Plus 50 mg Tablet is a combination medicine used to treat essential hypertension (high blood pressure). It contains Losartan Potassium, an angiotensin II receptor blocker (ARB), and Hydrochlorothiazide, a thiazide diuretic, which work together to lower blood pressure effectively.
Uses & Indications
Dosage
Adults
One tablet (Losartan 50 mg/Hydrochlorothiazide 12.5 mg) once daily. The maximum recommended dose is Losartan 100 mg/Hydrochlorothiazide 25 mg once daily, but this specific tablet strength allows for 50mg/12.5mg.
Elderly
No initial dosage adjustment required for elderly patients with normal renal function.
Renal_impairment
Dosage adjustment may be needed for severe renal impairment (CrCl <30 mL/min). Not recommended for anuric patients.
How to Take
Oral administration, usually taken once daily with or without food.
Mechanism of Action
Losartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively blocking the binding of angiotensin II to the AT1 receptor. Hydrochlorothiazide inhibits sodium reabsorption in the distal convoluted tubules, leading to increased excretion of sodium, chloride, and water, thereby lowering blood volume and blood pressure.
Pharmacokinetics
Onset
Losartan's peak antihypertensive effect is seen 6-9 hours post-dose. Hydrochlorothiazide's diuretic effect begins within 2 hours, with peak effect in 4-6 hours.
Excretion
Losartan and its metabolites are excreted via urine and feces. Hydrochlorothiazide is primarily eliminated by renal excretion.
Half life
Losartan has a terminal half-life of about 2 hours, and its active metabolite has a half-life of 6-9 hours. Hydrochlorothiazide has a half-life ranging from 5.6 to 14.8 hours.
Absorption
Losartan is well absorbed orally, with an absolute bioavailability of about 33%. It undergoes extensive first-pass metabolism by cytochrome P450 enzymes to form an active carboxylic acid metabolite. Hydrochlorothiazide is absorbed after oral administration, with an onset of diuretic action within 2 hours.
Metabolism
Losartan is extensively metabolized by CYP2C9 and CYP3A4 to an active metabolite (E-3174) and several inactive metabolites. Hydrochlorothiazide is not metabolized and is excreted unchanged.
Side Effects
Contraindications

- •Concomitant use with aliskiren in diabetic patients
Drug Interactions
Lithium
Increased serum lithium concentrations.
Aliskiren
Contraindicated in diabetics.
Other antihypertensives
Additive hypotensive effect.
Potassium-sparing diuretics/K supplements
Increased serum potassium.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
Reduced antihypertensive effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypotension, tachycardia, bradycardia (less common), electrolyte disturbances. Treatment is supportive and symptomatic.
Pregnancy & Lactation
Contraindicated in the second and third trimesters of pregnancy due to fetal toxicity. Not recommended during lactation as both drugs can pass into breast milk.
